Overspeed Trip of 2B Auxiliary Feedwater Pump During Surveillance

Description

A finding of very low safety significance and an associated NCV of 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures and Drawings, was self-revealed when the 2B diesel-driven auxiliary feedwater (AF) pump tripped on overspeed during a quarterly inservice test (IST). Specifically, operators with portable instrumentation used an erroneous speed value to adjust pump speed beyond the range specified in the procedure resulting in a pump overspeed trip, entry into a 72-hour technical specification (TS) required action statement, and unplanned pump unavailability with an associated change in Unit 2 risk from green to yellow.
