Description | Unit 2 Technical Specification 6.4.1(a) requires that written procedure be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Quality Assurance Program Requirements (Operation), Revision 2, February 1978. Section 9(a), Procedures for Performing Maintenance, of Appendix A of Regulatory Guide 1.33 states that procedures for maintenance that can affect the performance of safety-related equipment should be planned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances. Contrary to the above, the licensee failed to have adequate procedural guidance for disassembly and reassembly of the Unit 2 emergency diesel generator exhaust system wye piece. Specifically, the procedure failed to identify specific clearance tolerances due to the cast iron to carbon steel connection to prevent cracking of the cast iron exhaust wye piece. The performance deficiency of not providing adequate procedural guidance for maintenance on the Unit 2 emergency diesel generators, was determined to be more than minor because it was associated with the procedure quality attribute and adversely affected the Mitigating System cornerstone objective to ensure availability, reliability, and capability to respond to initiating events to prevent adverse consequences. Using Manual Chapter 0609, Attachment 0609.04, Initial Screening and Characterization of Findings, the finding was determined to be of very low safety significance because: (1) it was not a design or qualification deficiency, (2) did not represent a loss of safety function, (3) did not represent and actual loss of safety function of a single train for longer than its technical specification allowed outage time, (4) did not represent an actual loss of safety function of one or more non-technical specification trains, and (5) was not potentially risk significant due to seismic, flooding, or severe weather initiating event. The issue was placed into the corrective action program as Condition Report CR-ANO-2-2010-2706. |
