Title

Failure to Maintain Fire Watch and Fire Watch Logs

Description

Inspectors identified a Green, Severity Level IV, non-cited violation of 10 CFR 50.9(a), Completeness and Accuracy of Information, for the licensees failure to maintain the required fire watch logs complete and accurate in all material respects. The licensee entered this into their corrective action program as Condition Reports (CR) 2014-06416 and 2014-06680. This finding is more than minor because it adversely affected the human performance attribute of the Mitigating Systems cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. This finding has very low safety significance (Green) because it did not impact the ability to achieve safe shutdown. This findings severity level is based on an example in the Enforcement Policy, Section 6.1.d.2, which states, in part, that Severity Level IV violations involve violations of 10 CFR 50.59 [which] result in conditions evaluated as having very low safety significance.
