SUPPLEMENTAL INFORMATION

1. A 100% visual inspection of the reactor coolant system pressure boundary, where insulation was removed, was conducted. No other indications of '

unauthorized work were detected.

2. All uninsulated Class 2 piping connected to the reactor coolant system inside containment has had a 100% visual inspection. No indication of unauthorized work was detected.
3. Construction and contractor supervisors n d PGandE Nuclear Plant Operations personnel were informed of the incident in order to increase general awareness and to promote watchfulness for potential similar events.
4. The PGandE/Bechtel Project increased supervision of all construction.

activities within the Protected Area.

5. Supervision will continue to promote increased awareness of specfic work tasks in progress and the specific individuals assigned to each work task.
6. The Security Department increased the frequency of patrols within the work force areas in order to increase the visibility of security fortes.
7. Construction personnel hare been and will continue to be informed of job security as related to work schedules in order to minimize the possibility of deliberate acts to extend work.
8. Investigation is continuing in an effort to identify the individual or individuals responsible for this incident.
