Facility Operating License No. DPR-61 Docket No. 50-213 Supplemental LER 50-213/89-20-01 This le;tter forwards Licensee Event Report (LER) 89-20-01. This supplemental LER updates information vertaining to the fuel failures that were discovered during the 1989 refueling) outage and identified as a potential issue in the recent NRC's Historical Review Report . Whereas the original LER(2) presented a snapshot in time, this supplemental LER updates this information, presents the total number of discovered fuel failures based on completion of the fuel recovery program and provides additional detai' on the radiological assessment performed. This update also addresses the waste gas accident described in the Facility Description and Safety Analysis and its use of the 1% failed fuel assumption which is pertinent to the question raised by the NRC in their Historical Review Report.

Much of the new information provided has been extracted from reports prepared following completion of the refueling outage. These reports documented the Cycle 15- k i 16 fuel recovery efforts and provided details of the fuel inspections, the fuel damage k assessment, debris removal campaign and the fuel recovery and reconstitution. The conclusion from the original LER remains unchanged - that the fuel failures resulted in no safety significance and had no impact on the health and safety of the public.
