During normal steady state Mode 1 operation, at 100 percent power and during an In-plant Quality Assurance audit of the Emergency Plan, it was discovered that, contrary to Technical Specification 6.5.1.6.j, a change to the Emergency Plan had been issued without PORC review.

The root cause of this occurrence was a lack of adequate corporate level administrative controls over the distribution of revisions to the Emergency Plan.

Investigation of the root cause revealed a potential for the same error to occur with the revision of similar manuals requiring PORC review.

The Emergency Plan change was subsequently reviewed by PORC on March 12, 1986.

As a result, an errata to the revision was issued on March 21, 1986 to address PORC comments and to bring the Emergency Plan into Technical Specification compliance. Corporate level administrative controls will be revised to ensure all Technical Specification required reviews will be performed orior to distribution of documents. Personnel responsible for compliance with and administration of these controls will be instructed in the use of these new controls.

These actions will be completed by August 1,1986. Meanwhile, interim administrative controls have been put in place. There was no adverse effect on the health and safety of the public as a result of this event. gg
