On September 30, 2021, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Wolf Creek Generating Station. On October 7, 2021, the NRC inspectors discussed the results of this inspection with Mr. Jaime McCoy, Site Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.

NRC inspectors documented one finding of very low safety significance (Green) in the report that did not involve a violation of NRC requirements.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection and an independent spent fuel storage installation inspection at Wolf Creek Generating Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. List of Findings and Violations

Failure to Implement Critical Design Parameters for the Main Feedwater System During Digital Control System Modifications Cornerstone Significance Cross-Cutting Report Aspect Section Initiating Events Green [H.12] - Avoid 71153 FIN 05000482/2021003-01 Complacency Open/Closed The inspectors reviewed a self-revealed, Green finding associated with the licensees failure to ensure balance of plant control system vendor used critical design parameters for the nonsafety-related main feedwater system bypass regulating valves in accordance with Procedure AI 05-005, Modifications to Non-Controlled [structures, systems, and components], Revision 5. PLANT STATUS

Wolf Creek Generating Station began the inspection period at full power. On August 18, 2021, an automatic reactor trip from 100 percent reactor occurred due to a loss of main feedwater flow to the B steam generator caused by a stem separation of the B main feedwater regulating valve.

The licensee commenced Forced Outage 25-02 to repair the B main feedwater regulating valve and performed extent of condition reviews of the remaining three main feedwater regulating valves. On August 21, 2021, the unit commenced reactor startup, and the reactor was made critical. The unit was returned to full power on August 22,

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Starting on March 20, 2020, in response to the National Emergency declared by the President of the United States on the public health risks of the coronavirus (COVID-19), resident and regional inspectors were directed to begin telework and to remotely access licensee information using available technology. During this time, the resident inspectors performed periodic site visits each week, increasing the amount of time onsite as local COVID-19 conditions permitted.

As part of their onsite activities, resident inspectors conducted plant status activities as described in IMC 2515, Appendix D; conducted routine reviews using IP 71152, Problem Identification and Resolution, observed risk significant activities; and completed onsite portions of IPs. In addition, resident and regional baseline inspections were evaluated to determine if all or a portion of the objectives and requirements stated in the IP could be performed remotely. If the inspections could be performed remotely, they were conducted per the applicable IP. In some cases, portions of an IP were completed remotely and onsite. The inspections documented below met the objectives and requirements for completion of the IP.

===60856 - Review of 10 CFR 72.212(b) Evaluations

The inspection was conducted to determine if Wolf Creek Generating Station met the general license requirements of 10 CFR Part 72 before conducting operations to load their Independent Spent Fuel Storage Installation (ISFSI).

Review of 10 CFR 72.212(b) Evaluations===

(1) On August 16-19, 2021, the NRC conducted a team inspection of Wolf Creek Generating Station's 10 CFR Part 72 program implementation and 10 CFR 72.212 report prior to the licensee's first loading campaign, which is scheduled for the fall of 2021. Wolf Creek Generating Station has elected to become a 10 CFR Part 72 general licensee in accordance with 10 CFR 72.210. The licensee has selected the Transnuclear Americas LLC Nutech Horizontal Modular Storage Extended Optimized Storage (EOS) system. The licensee constructed the ISFSI storage pad in the spring of 2020 and built the EOS NUMHOMS in the fall of 2020, in accordance with the Transnuclear NUHOMS EOS Certificate of Compliance 72-1042, Amendment 1 and Final Safety Analysis Report, Revision 3.

The following programs, procedures, and calculations were inspected:

INSPECTION RESULTS

Failure to Implement Critical Design Parameters for the Main Feedwater System During Digital Control System Modifications Cornerstone Significance Cross-Cutting Report Aspect Section Initiating Events Green [H.12] - Avoid 71153 FIN 05000482/2021003-01 Complacency Open/Closed The inspectors reviewed a self-revealed, Green finding associated with the licensees failure to ensure balance of plant control system vendor used critical design parameters for the nonsafety-related main feedwater system bypass regulating valves in accordance with Procedure AI 05-005, Modifications to Non-Controlled SSCs, Revision 5.

Procedure AI 05-005 implemented the licensee self--imposed standard to control the main feedwater system as controlled plant equipment which required configuration-control toconform with Title 10 Code of Federal Regulations Part 50, Appendix B rules and regulations. As a result, operators observed oscillating and divergent water levels in all four steam generators that caused an automatic reactor trip.

Description:

During Refueling Outage (RF) 24, the licensee upgraded multiple nonsafety-related nuclear supply system control systems to the Ovation based distribution control system. These changes included new digital controls for the main feedwater system regulating valves and the associated bypass valves. The main feedwater system was a nonsafety-related SSC that was controlled in accordance with Procedure AI 05-005.

Procedure AI 05-005, Attachment A, classified the main feedwater system as controlled plant equipment. This designation implemented the self-imposed standard that configuration control for the main feedwater system conforms with 10 CFR Part 50, Appendix B rules and regulations. Therefore, the inspectors concluded the licensees self-imposed standard was to ensure modifications of the nonsafety-related main feedwater system to the Ovation based distribution control system was controlled in accordance with 10 CFR Part 50, Appendix B, Criterion III, which states, in part, that measures shall also be established for the selection and review suitability of application of material, parts, equipment, and processes that are essential to the safety-related functions of the structures, systems, and components.

On May 12, 2021, the licensee was performing a reactor startup following the completion of RF 24 when steam generator water levels began oscillating and diverging at approximately 8 percent reactor power while in automatic control. Operators took manual control of the main feedwater regulating bypass valves but were unable to restore levels prior to a steam generator low level automatic reactor trip. The cause of the automatic reactor trip was the low-power controller gains for the main feedwater bypass regulating valves were set too high. This led to a higher feedwater flow to the steam generators than needed for the feedwater system demand. This condition resulted in steam generator water level control instability causing oscillating and diverging water levels. The licensee entered this issue into their corrective action program as Condition Report 10005736, classified this a significant condition adverse to quality and performed a root cause evaluation. The root cause evaluation identified one root cause and one contributing cause associated with the automatic reactor trip that occurred on May 12, 2021. The root cause and contributing cause are as follows:

Cause:

The licensees specific main feedwater bypass regulating valve characteristics and the impact of the feedwater control system designed were not fully recognized. This resulted in the incorrect control system tuning parameters and inadequate risk mitigation strategies for the initial power ascension following the implementation of the Ovation main feedwater digital control system. Specifically, the licensees vendor did not incorporate the stations main feedwater bypass regulating valve critical design parameters of a 4-inch bypass valve with a maximum flow coefficient of 150 for determining the initial tuning parameters for the valves during the Ovation upgrade. Instead, the vendor used a 6-inch bypass valve with a flow coefficient near double the value of the stations bypass valve flow coefficient. This resulted in the low-power controller gain values for the main feedwater bypass regulating valves being set too high. These parameters resulted in the steam generator water level control instability causing oscillating and diverging water levels, resulting in the May 12, 2021, automatic reactor trip.

Contributing cause: Throughout the Ovation modification design and implementation process, there was inadequate risk recognition and advocacy which resulted in missed opportunities for leaders to engage cross-functionally and make key decisions. For example, the licensees initial just-in-time training for the Ovation system modification was based on an analysis of training needs and focused on performing the reactor startup activities with the Ovation system. However, the licensee concluded the possible failures of all main feedwater bypass regulating valves, with oscillating and diverging steam generator water levels at low power, was not determined to be a likely failure even though it occurred on the plant training simulator. This similar event occurring on the plant training simulator provided the licensee with an opportunity to validate the vendor used the correct main feedwater system critical design parameters and their conclusion the plant training simulator was not comparable to the anticipated plant response when implementing the new main feedwater digital control system.

The inspectors reviewed the above information and concluded the licensee failed to implement the self-imposed standard to control the main feedwater system as a controlled plant equipment in accordance with Procedure AI 05-005. Specifically, the licensee failed to ensure the balance of plant control system vendor used critical design parameters for the main feedwater system bypass regulating valves.

Corrective Actions: The licensee implemented the following immediate corrective actions: 1. The initial controller gain values were updated for the main feedwater bypass regulating valves using the stations critical design parameters prior to commencing reactor startup on May 14, 2021; 2. The licensee performed additional just-in-time training to ensure licensed operators were familiar with manual digital control system for the main feedwater bypass regulating valves prior to the May 14, 2021 reactor startup. The corrective action to prevent recurrence was to update design drawings with a caution that a change to the valve critical design parameters could impact the Ovation feedwater control logic.

Performance Assessment:

Performance Deficiency: The licensees failure to ensure balance of plant control system vendor used critical design parameters for the main feedwater system bypass regulating valves was a performance deficiency.

Screening: The inspectors determined the performance deficiency was more than minor because it was associated with the Design Control attribute of the Initiating Events cornerstone and adversely affected the cornerstone objective to limit the likelihood of events that upset plant stability and challenge critical safety functions during shutdown as well as power operations. Specifically, the licensees failure to use the main feedwater system bypass regulation valve critical design parameters during balance of plant control system modification resulted in oscillating and divergent water levels in all four steam generators resulting in an automatic reactor trip.

Significance: The inspectors assessed the significance of the finding using Appendix A, The Significance Determination Process (SDP) for Findings At-Power. Using Exhibit 1, Section B.

The inspectors determined the finding was of very low safety significance (Green) because the finding did cause a reactor trip, but not the loss of mitigation equipment relied upon to transition the plant from the onset of the trip to a stable shutdown condition (e.g., loss of condenser, loss of feedwater).

Cross-Cutting Aspect: H.12 - Avoid Complacency: Individuals recognize and plan for the possibility of mistakes, latent issues, and inherent risk, even while expecting successful outcomes. Individuals implement appropriate error reduction tools. Specifically, the licensee failed to validate the vendor design control parameters for the main feedwater system bypass regulating valves and reevaluate the risk associated with oscillating and diverging water levels in all four steam generators, even when plant training simulator indicated the potential of this event to occur.

Enforcement:

Inspectors did not identify a violation of regulatory requirements associated with this finding.

On July 28, 2021, the inspectors presented the cyber security problem identification and resolution inspection results to Mr. L. Lane, Director Strategic Engineering and other members of the licensee staff.
