Section I - Part B OBJECTIVES FOR THE 1982 KEWAUNEE V NUCLEAR POWER PLANT EMERGENCY PREPAREDNESS EXERCISE ,

In order to demonstrate the emergency preparedness of the Kewaunee Nuclear Power Plant and the Wisconsin Public Service Corporation, an emergency preparedness exercise will be conducted. The primary ' objective of the exercise is to demonstrate the ability to implement the Kewaunee Nuclear Power Plant Emergency Plan which has been developed to ensure an adequate level of preparedness and an effective response to emergencies at the Kewaunee Nuclear Power Plant. Specific objectives designed to further define the main objective are as follows:

A. Accident Assessment

1. Demonstrate the ability of personnel to recognize an emergency initiating event and properly characterize'and classify the emergency according to the pre-established Emergency Action
Levels.

2. Demonstrate that personnel can perform radiological accident assessment and dose projection techniques by using plant monitor l readings , environmental data, and ' meteorological parameters.

3. Demonstrate that environmental monitoring teams can be dispatched and deployed in a timely manner; that communications are adequate; that radiological monitoring equipment is functional; that simu-() lated data are accurately obtained and transmitted to appropriate emergency response facilities.

4. Demonstrate proficiency in the assessment of plant conditions and (v) t radiological consequences in determining appropriate procedures to be used in response to and recovery from an emergency; demon-strate that results can be effectively used in determining protec-tive action recommendations.

i Section I - Part F Page 1 GUIDELINES FOR CONDUCT OF 1982 KEWAUNEE NUCLEAR POWER PLANT EMERGENCY PREPAREDNESS EXERCISE I. PURPOSE This document provides guidance for the conduct of the 1982 Kewaunee Nuclear Power Plant emergency preparedness exercise. It provides specific methods for demonstrating emergency response . capability, conducting the exercise, and evaluating results.

II. CONCEPT OF OPERATIONS AND CONTROL OF THE EXERCISE Wisconsin Public Service Corporation (WPS) will provide for official refer-ees and/or observers for each location-where an emergency response is being demonstrated for the exercise. Prior to the exercise, the referees and observers will be provided with the appropriate locations, maps, time periods, guidelines, and an observer evaluation checklist for.their exer-cise assignments.

The exercise initiating events will be controlled by lead referees at the Kewaunee Nuclear Power Plant. Hypothesized initiating events will consist of three types of information: (1) plant-information and data provided to Control Room and Technical Support Center personnel; (2) release rates of Noble Gas and Iodine; (3) hypothesized' environmental radiation levels. The lead referees have the authority-to determine the time. sequence of these initiating events to ensure an orderly flow of exercise events. All other actions during the exercise will occur through a free-play response as?the plant and corporate participants respond to the initiating events.

As the initiating events are provided to the plant staff, they will deter-mine the nature of the emergency and the. implementation of the' appropriate emergency response procedures. These procedures are expected to include a determination of the emergency classification in accordance with Kewaunee Nuclear Power Plant Emergency Plan. After the emergency classification has been determined, the ' appropriate Federal, State, and county authorities will' be notified in accordance with the Emergency Plan Implementing Proce-dures.

Upon notification of the hypothesized emergency at Kewaunee Nuclear Power Plant, the State of Wisconsin, Kewaunee County, and Manitowoc County will acknowledge their notifications in accordance with their' emergency plans and procedures. ,

The hypothesized emergency will continue.to develop based on data and information provided to the Control Room and TSC personnel. As the situ-ation develops, information will be forwarded to appropriate plant and-corporate personnel for the determination of actions to -be f taken in response to the hypothesized emergency. Where information would normally be confirmed via an independent source'(such as National Weather Service for weather data),-the confirmation data will be simulated. If a conflict exists between the hypothesized data and the actual data, the hypothesized data g% g will be utilized.

If any inconsistencies are noted in the initiating events, they should be questioned by the plant and corporate personnel as they would in a real emergency. Certain inconsistencies (such as plume width, release duration, technical reason for the simulated release, etc) may be intentional to provide an exercise basis which tests the plant and corporate capabilities to the maximum extent feasible in a limited time frame. If an inconsis-tency is known or determined to be intentional, then the emergency response personnel will note the inconsistency and ignore it. The lead referees shall have.the_ authority to resolve or explain any inconsistencies or problems that may occur during the exercise.

With the exception of the aforementioned cited and potential inconsistencies, the internal operations of the emergency response facilities shall be identical with their intended operation in a real emergency.

III. GENERAL GUIDANCE FOR THE CONDUCT OF THE EXERCISE

1. Simulating Emergency Actions Since the exercises are intended to demonstrate actual abilities as closely as is reasonably possible, the participants should act as they would during a real emergency. All actual actions shall be taken where possible, with demonstrations utilized only when it is not feasible to perform the actual action in accordance with other guidelines.

When orders are given to others during drills and exercises, the orders (gj -

f-s should contain the word " Simulate" as the first word, if that is what is meant. For example, the order to put out a fire that is being hypothesized would state: " Simulate discharging the fire extinguisher." When such actions are being simulated, it is suggested that participants inform any observers in the area of what action would actually be taken had the emer-gency been real.

2. Avoiding Violation of Laws Intentional violation of lawg is not justifiable during any exercise. To implement this guideline, the following actions must be taken prior to the conduct of all exercises:
a. All observers and potential exercise participants must be specifi-cally informed of the need to avoid intentional violation of all Federal, State, and local laws, regulations, ordinances, statutes, and other legal restrictions. The orders of all police, sheriffs, or other authorities should be followed as would normally be the l case.

b. Exercise participants shall not direct illegal actions being taken by other exercise participants or members of the general public, t
c. Exercise participants shall not intentionally take illegal actions when being called out to participate in an exercise. Specifically, local traffic laws such as speed laws shall be observed.

3. Avoiding Personal and Property Endangerment Participants and observers shall be instructed to avoid endangering prop-erty (public or private), other personnel responding to the exercise, members of the general public, animals, and the environment.
4. Action to Minimize Public Inconvenience It is not the intent, nor is it desirable or feasible, to effectively train or test the public response during the conduct of radiological emergency exercises. Public inconvenience is to be minimized.

The actions of Federal, State, and county agencies and nuclear power plant operators receive continuous public notice and scrutiny; therefore, the conduct of an exercise could arouse public concern that an actual emergency is occurring. In order to minimize public inconvenience and concern, the following actions are being included in the exercise plans:

a. A few days prior to the exercise, the public within the affected area will be informed of the impending conduct of a radiological emergency exercise. Press releases from Wisconsin Public Service Corporation will be used to accomplish this. Prior to the exercise the public shall be informed of the following as a minimum:

(1) The purpose of the exercise and that it is a routine periodic function not intended to inconvenience the public unnecessarily.

(2) The exercise requires no public action or involvement.

(3) The exercise will be conducted at a preselected time during a specifically stated time period.

(4) Typical actions of Federal, State, and local authorities during the exercise.

b. During the exercisq, an announcement will be provided by WPS stating that an exercise is in progress,
c. Agencies should make every effort to prevent the public from being misinformed about the nature of the exercise. Therefore, it is important to inform members of the public that an exercise is in progress and that their involvement is not required, whenever direct public contact occurs in tlIe field. In addition, it is important to preface, interject, and conclude conversations that can be overheard by the public with the words, "This is an exer-cise, repeat, this is an exercise" or similar statements. Conver-sations that can be monitored by the public include those on telephones, radios, loudspeakers, telegraphs, etc.
5. Maintaining Emergency Readiness During the performance of an exercise the ability to recognize a real

, / cmergency, terminate the exercise, and respond to the new situation must be maintained. Therefore, the exercise scenario and actions of participants i

i Section I - Part F Page 4 f, %} shall not include any actions which seriously degrade the condition of systems, equipment or supplies, or affect the detection, assessment, or response capability to radiological or other emergencies.

Actions taken by the participants shall also avoid actually reducing plant or public safety. The potential for creating real radiological or other emergencies shall be specifically avoided.

If a real emcrgency occurs during the exercise, requiring the actions of plant and corporate personnel, then the exercise shall be terminated. All messages about real events shall be clearly identified as such. For example, precede a real message with "This is NOT, repeat, NOT an exercise message."

IV. EMERGENCY RESPONSE IMPLEMENTATION AND OPERATIONS

1. Initial Notification Initial notification shall be performed in accordance with the appropriate Emergency Plan Implementing Procedures (EPIPs).
2. Activation of Facilities Activation of State and county Emergency Operation Centers (EOCs) and manning of the EOCs by emergency response personnel shall be simulated.

3. Plant Operations It should be emphasized that the exercise will in no way inter-rupt normal plant operations, production and safety. The responses of control room personnel and plant operators shall be simulated when taking action to combat the emergency. Instead, a referee will narrate the initiating events and the postulated plant response directly to the Shift Supervisor and other control room personnel who will then employ the appropriate emergency pro-cedures. ,
4. Environmental Monitoring Teams WPS will deploy environmental mon'itoring teams. The referees will provide hypothesized field data which will be used to determine radiation readings at preselected locations. However, these teams will be equipped with the ne,cessary equipment which would enable them to determine actual area gamma dose rates and air-borne radioiodine concentrations. The monitoring teams will not be suited up in anticontamination clothing, but will have such equipment at their disposal.
5. Public Notification The State of Wisconsin and Kewaunee County and Manitowoc County emergency response plans have prepared text messages for informing

,j the public of a nuclear power plant incident. These messages include instructions on protective actions and where to get additional emergency information. For the purposes of this

\ exercise, the state and county agencies will not participate except to log in the time.of receipt and contents of the messages' from Wisconsin Public Service Corporation.

6. Public Information Press releases to the general public and' news media'shall be simulated. The simulated press releases shall contain all neces-sary information on the. current status of the exercise, but will not be transmitted.

7. Communications Communications between the exercise participants shall occur in accordance with the appropriate Emergency Plan Implementing Pro-cedures. There will not be a simulation of communication equip-ment failures. However, should any primary communication path become inoperative or prove inadequate, backup means of commu-nication will be utilized as appropriate.

V. CLOSE0UT OF THE EXERCISE

1. The following plant status conditions will serve as a general guideline for initiating the closeout of the exercise and.estab-lishing a recovery organization:

() a. Radiation levels are stable or decreasing with time,

b. Release of radioactive materials to the environment has ceased or is controlled within permissible license limits.
c. Fire or similar emergency conditions no longer constitute a hazard to.the plant or plant personnel.

ABSTRACT OF KEWAUNEE EXERCISE SCENARIO At 0820, a fire occurs in the Reserve Auxiliary Transformer and causes the loss of bus 1-6. Diesel Generator IB starts and picks up bus 1-6 through the automatic sequence. Because of-the voltage fluctuation, the Main Generator trips on the Single Channel Overspeed Trip System (SCOTS). Buses 1-1, 1-2, 1-3, and 1-4, which are being supplied from the Main Auxiliary Transformer, are lost with the Main Generator. With the loss of these buses, the Reactor Coolant Pumps and Main Feed Pumps are lost. The Reactor trips on loss of flow. Charging Pump 1B is available and being used to maintain pressurizer level. An unusual event is declared when fire lasts more than ten minutes.

At 0832, the fire is out in the Reserve Auxiliary Transformer.

At 0835, buses 1-5 and 1-6 are cross-connected to the Tertiary Auxiliary Transformer. When this is completed, the diesel is secured. Shortly after restoration of bus 1-52, Charging Pump 1A breaks a drive belt.

At 0845, a ground develops in the Tertiary Auxiliary Transformer and it locks out, causing the loss of buses 1-5 and 1-6. Diesel Generator 1-B restarts and picks up bus 1-6 and regains Charging Pump IB. The Steam-Driven Auxiliary Feed Pump will not operate.

At 0915, 1B Auxiliary Feedwater Pump trips on overcurrent. Steam pressure is decreasing, and Reactor Coolant temperature is increasing, indicating a very low level in the steam generators. The Power-Operated Relief Valves (PORVs) lift and limit pressure to 2,335 lb. Steam pressure continues to fall'and safety injection is initiated. A Site Emergency is declared with the loss of all feedwater.

An attempt to correct the problem with IB Auxiliary Feed Pump fails.

At 1030, laboratory analysis confirms fuel damage.

At 1045, an operator in the Boric Acid Evaporator Room is injured and contaminated, requiring first ' aid and assistance.

At 1050, the PORVs are opened to reduce pressure and gain safety injection flow, but they do not work in manual control. The Residual Heat Removal System (RHR) lineup is started.

At 1210, there is rapid depressurization of tihe Reactor Coolant System, followed by high radiation readings on the Auxiliary Building Vent Moni-tors. A General' Emergency should be declared. Eventually, a break is discovered in the RHR piping and is isolated. The release is terminated, but the Reactor Coolant System is now depressurized. An alternate lineup of the 'RHR system is made, bypassing the ruptured portion of piping. RHR is started and the plant goes into the recovery phase. Radiation levels begin to fall, and the exercise is terminated at 1530.

p INITIAL CONDITIONS O The plant has been operating at 100 percent power for 80 days. No. IA Diesel Generator is out of service for repair of a generator ground, and it is estimated to be back in service in 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. The plant is in a normal electrical lineup, with buses 1-1, 1-2, 1-3, and 1-4 supplied from the Main Auxiliary Transformer; bus 1-6 supplied from the Reserve Auxiliary Trans-former; and bus 1-5 supplied from the Tertiary Auxiliary Transformar. No releases are taking place. SI302B is shut and isolated electrics 11y because of a leak in check valve SI303B. Boric Acid Evaporator is in operation.

T= 0815 Plant conditions normal.

T= 0820 Fire in Reserve Auxiliary Transformer, loss of bus 1-6. Diesel Generator IB starts and picks up bus 1-6. Main Generator trips from (SCOTS). Loss of Main Auxiliary Transformer and buses 1-1, 1-2,1-3, and 1-4 and subsequent loss of Reactor Coolant Pumps and Main Feed Pumps. Reactor trips on loss of flow. Charging Pump 1B is being used to maintain pressurizer level.

T= 0832 Fire is out in Reserv"e Auxiliary Transformer.

      • Unusual Event declared ***

(Fire lasts more than 10 minutes)

T= 0835 Operator cross-connects buses 1-5 and 1-6 to the Tertiary Auxil-iary Transformer, and the Diesel is' secured.

T= 0836 Upon restoration of 480-V bus 1-52, Charging Pump 1A breaks a drive belt.

T= 0845 A ground develops in the Tertiary Auxiliary Transformer causing it to lock out and resulting in the loss of buses 1-5 and 1-6.

Diesel Generator 1-B restarts and picks up bus 1-6 with Charging Pump 1-B. To limit the cool-down rate, the Steam-Driven Auxiliary Feed Pump is disabled at the steam supply valve.

Shift Supervisor determines if additional staff augmentation is l necessary and Emergency Response Facilities should be activated.

T= 0915 Steam pressure is decreasing, and Reactor Coolant temperature is to increasing, indicating a very low level in the steam generators.

0940 Reactor coolant pressure is increasing and the PORVs lift and limit pressure to 2,335 lb. When steam pressure falls to 500 lb.,

safety injection is initiated. A reactor coolant sample is taken. An attempt is made to start the Steam-Driven Auxiliary Feed Pump, but its throttle valve trips.

      • Site Emergency ***

ESF anomaly In an attempt to correct the problem with Auxiliary Feedwater Pump 13, the breaker is removed from 1A and installed in IB cubicle. When starting, 1B again trips, confirming the fault to be in the pump motor. Loss of both motor-driven auxiliary feed-water pumps and loss of supply steam to the Steam-Driven Auxiliary Feedwater Pump constitute a complete loss of feedwater flow.

T= 1030 Laboratory analysis of a Reactor Coolant sample shows greater than 300 uci/ml of I 131 equivalent and confirms fuel damage.

T= 1035 SI302B is opened in preparation for residual he.-t removal (RHR).

SI302 must be electrically aligned at the MCC.

T= 1045 While placing the Boric Acid Evaporator in shutdown, following

T= 1050 Operator discovers that he does not have manual control of the PORVs. These continue to operate in the automatic mode and limit Reactor Coolant System pressure to 2,335 lb.

T= 1130 - 1200 Lunch T= 1210 Rapid depressurization of the Reactor Coolant System occurs.

T= 1315' Operators isolate ruptured portion of RHR system by closing valves RIDI 7B and RHR 9B.

T= 1345 RHR is initiated by starting RHR pump 1B and establishing a flow path through RHR 100B, FGUI 100A, Residual Heat Exchanger IA, RHR 10A, RER 10B, and SI302B.

T= 1400 When radiation levels fall below the limits .of Figure EP-AD-2.1 or EP-AD-2.2, the condition de-escalates from a general emergency .

to a site emergency.

T= 1415 Diesel Generator 1A is placed back in service.

For the purposes of the exercise, time is compressed to achieve the following conditions:

When the auxiliary feedwater system is restored, or the Reactor Coolant System temperatures fall below 350 F with RHR, and radi-ation levels are reduced below the site emergency limits of Figure EP-AD-2.1 or EP-AD-2.2, the condition de-escalates from a site emergency to an alert.

The condition will not be reduced below alert for the duration of the exercise because of the indicated fuel damage.
