However,' the violation did 'not. affect the health and safety of _ the public.

II. REASON FOR THE VIOLATION-

The guidance provided in:PlantLAdministrative Procedure 01-S-06-2,

" Conduct.of Operations", was inadequate.. It was not clear when a

. procedure change or just written ' instructions were required to manipulate valves in a - system.- After encountering operational. difficulties, the ^

1 shift. supervisor provided handwritten-directions'for placing'the heater ~

. drain pumps in the pump forward mode. He erroneously considered his  ;

actions to be within the. latitude provided by the " Conduct of Operations"

~

procedure.

III.; CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED e .

Plant Administrative Procedure'01-S-06-2, " Conduct of Operations", was revised. ;This procedure now gives clear direction that only a written 3

approved' procedure shall be used when operating plant systems.

IV.-CORRECTIVE ~ STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS '
