INTRODUCTION k

1. Volumetric, surface and visual examinations were performed as required by Section XI, of the ASME Boiler and Pressure Vessel Code, j 1980 Edition, including the Winter 1981 Addenda.

2. All records, examination data sheets, personnel certificates, equipment and material certificates for the examinations performed are on file at the Millstone Nuclear Power Station Unit #2.

3. All items listed in this report are creditable items to the second interval in-service inspection ten-year Class I and Class I program, with the exception of the preservice examination of four reactor c

vessel head closure studs and the additional examinations required by IWB-2430. *

4. The examinations listed in this report were performed by personnel from the Northeast Utilities Service Company, Nuclear Engineering and Operations Group and EBASCO Services Incorporated.

5. In addition to the second interval creditable items listed above, several items were examined that are creditable to the first ten-year in-service program. These items will be noted throughout the content of this report by specific notations relative to those exams.

1. The Millstone Unit #2, 1986 Refueling Outage was the first outage of the first period in the second Ten-Year Interval. There were no 4 exclusions, exceptions, or deferred items during this outage.

2. The ASME Section XI, Code Category B-F, Trimetalic Welds that were

deferred during the 1985 refueling outage of the first Ten-Year interval were examined.

1. Volumetric, surface, and visual examinations were performed as required by Section XI of the ASME Boiler and Pressure Vessel Code, j 1980 Edition, including the Winter '81 Addenda. However, the extent of examinations for Class 1, (B-J) and Class 2 (C-F & C-C), are still determined by the 1974 Edition, including the Summer '75 Addenda as required and/or permitted by 10CFR50.55a.

2. Four reactor vessel closure head studs were replaced this refueling outage. See Section VI, Category B-G-1, for Preservice Examination Results.

/""% 3. Category B-J, Item B9.40, Socket Weld Examinations were expanded by U five additional welds due to unacceptable indications discovered in Weld BPD-C-1015. These additional welds were selected in accordance with the requirements of IWB-2430(a) of the applicable code.
