WOLF CREEK: SUSPECTED FALSIFICATION OF RECORDS j (4-85-011)  !

1 Enclosed is the above captioned Report of Investigation. This investigation ,

was initiated after the completion of case' A4-85-005, wherein the Quality First (Q1) investigative program of Kansas Gas and Electric (KG&E) at the Wolf .

Creek Generating Station, was reviewed for completeness and adequacy. This case, initially investigated by the Q1 office, was selected to be further examined by 01 due to the appearance of undeveloped leads and questionable .

findings in an effort to determine if the matter was adequately investigated by Q1.

An alleger initially reported to Q1 that a Technical Metals Incorporated (TMI) project engineer had failed to conduct required inspections and subsequently falsified inspection records. The Q1 investigator determined the inspections in question and corresponding records dealt with the exterior surface cleanliness of non-Q piping within containment at Wolf Creek. Although the Q1 investigator neglected to examine the records in question or interview the project engineer,  ;

he documented the alleger's allegation as substantiated. The investigator closed the investigation explaining the records were not directly related to a j safety system.

01's investigation identified seven inspection memos signed by the project i engineer which were allegedly falsified. An interview of the project engineer )

verified that this individual did not always visually inspect the external cleanliness of the non-Q pipe. The project engineer admitted he subsequently falsified inspection documentation to conceal his lack of inspection. It is-01's conclusion that the Q1 investigation was not conducted in a manner that would meet acceptable investigative standards of the NRC regarding completeness and adequacy. ]

The NRC Region IV resident inspector's evaluation of the falsified inspection documents, and the related piping, revealed the areas in question were all i contained within the reactor building and all dealt with "non-nuclear safety" piping.
