In accordance with Appendix A, Beaver Valley Technical Specification 3.7.15 " Fire Rated Assemblies", the following Special Report is submitted. This report is required when the conditions specified in Technical Specification 3.7.15.a are not satisfied.

On June 16, 1986, during the performance of Beaver Valley's Fire Rated Assemblies Visual Inspection, Beaver Valley Test 1.3-1.33.5, three (3) fire dampers (VS-D-93, 95A, and 95B) were declared inoperable after not fully closing during testing. A fire watch was already in effect in the area due to construction activities. Fire detection in the area was verified to be operable. A Maintenance Work Request was initiated. The dampers were determined to be binding and required minor mechanical adjustments and lubrication. On July 7, 1986, after successful re-testing, the dampers were returned to service. There were no safety implications due to this event as the fire detection system remained operable at all times and the fire watch remained in effect throughout this event.
