APPENDIX A NOTICE OF VIOLATION 1~

Louisiana Power & Light Company Docket: 50-382 Waterford Steam Electric Station, Unit 3 License: NPF-38 During an NRC inspection conducted on June 1-30, 1986, violations of NRC requirements were identified. These violations involved failure to take prompt corrective action to bring valves into compliance with equipment qualification program requirements and failure to follow written approved procedures. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2. Appendix C (1985), these violations are listed below:

A. Technical Specification (TS) 6.8.1 endorses NRC Regulatory Guide 1.33, Revision 2, which in turn endorses ANSI h18.7, 1976. ANSI N18.7, 1976

' requires that modifications of equipment shall be preplanned and perfonned in accordanc.e with written procedures. PE-2-006, Revision 7. " Station

!@!=jifications," is an approved procedure which specifies the requirements

.or implementation of mooifications to safety-related plant structures, systems, and components, and particularly in Section 5.9.5 requires that the action engineer redline any control room drawings affected by a modification.

Contrary to the above, for Station Modification 896, "CCW Surge Tank Vent Line Isolation," the appropriate control room drawing was not redlined.

This is a Severity Level IV violation. (SupplementI)(50-382/8613-01) i B. 10 CFR Part 50, Appendix B, Criterion XVI, which is implemented by Section 17 of the Louisiana Power & Light Nuclear Operations Quality

, Assurance Program, requires, in part, that, " Measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment,

. and nonconformances are promptly identified and corrected."

Contrary to the above, prompt action was not taken to correct deficiencies in that, on April 9, 1986, valves SI-225B and SI-227B were identified as possibly not meeting the requirements of the licensee's equipment qualifi-cations program which implements the requirements of 10 CFR Part 50.49.. j Yet, required corrective actions to bring the valves into compliance were not taken until June 18, 1986.

Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if admitted, 8607290145 860724 PDR ADOCK 05000392

t (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
