' ~ This refers tc the inspection conducted November 3 through December 23,19g7, at the i

Quad Cities Nuclear Power station. The purpose of the inspection was to determine whether

activities authorized by the license were conducted safely and in accordance with NRC

'

requirements.1 During this period routine resident inspections were performed. Based on the ,

-inspectcrs' findings, overall operation of the Quad Cities Station was safe. At the conclusion of '

.

. the inspection, the findings were discussed with those members of your staff identified in the

enclosed report.

Based on'the results cf this inspection, the NRC has determined that three violations of NRC ;

requirements occurrei The vi# ions are cited in the enclosed Notice of Violation (Notice) and

- the circumstances surrounding imm are described in detail in the subject inspection report.

The first violation involved the faluw s, rully carry out corrective actions in response to several  :

4 kV safety-related breaker problems identified at Quad Cities and another Commonwealth

Edison Company facility,- Planned corrective actions to replace safety-related breakers with

' refurbished breakers were not completed in a timely manner for all breakers despite evidence of-

breaker grease stiffening due to age hardening.- Other corrective actions intended to keep ,

breaker grease intact prior to refurbishment were also not completed in a timely manner. Some l

. preventive maintenance actions recommended by the breaker vendor were not performed. in j

[ . f some cases, breakers had not been overhauled for 28 years, even though they were known to 1

contain grosse susceptible to age harder.:ng. Your response to this violation should characterize 1

n  : the specific elements of your program which are in place to give confidence that breakers at l

. Quad Cities are acceptable for operation. You should also address the program you have in - -l

place to ensure that vendor information required for equipment operation is incorporated into 1

_ appropriate maintenance requirements. l

! l.The second violation is of concem because maintenance errors during. valve repairs were not

-Ld etected prior to operating the unit,' and corrective actions.did not appear to be effective to ' ,

y ensure similar problems would not recur.

LThi third violation, regarding missed Technical Specification surveillances, is of concem

because of the number of missed surveillances since the implementation of the new

noncompliances. However, two of the three examples in this violation were not discovered as a
