Dear Dr. Grace:

Carolina Power and Light Company (CP&L) provides this Special Report on the inoperability of the Motor Driven Fire Pump (MDFP) for more than seven days in accordance with Technical Specification (T.S.) 6.9.3.1.d pursuant to the requirements of T.S. 3.14.2.

Unit No. 2 was in cold shutdown with a refueling outage in progress. ,

Unit No. 2 MDFP was taken o'tt-of-service to permit divers to safely enter the circulating water (CW) pump intake structure from which the MDFP takes suction. The divers were needed to prepare the intake bay for dewatering. During this phase, the MDFP could be used in an emergency by removing the divers from the water.

On Thursday, February 6,1986, the intake bay was dewatered to perform additional inspections and maintenance. At this time, the MDFP no longer had a source of water. During the dewatering and inspection activities, delays were encountered which lengthened the time the MDFP was out-of-service.

On February 11, the CW intake bay was flooded. The MDFP was returned to service on Wednesday, February 12,1986, at 1105 hours0.0128 days <br />0.307 hours <br />0.00183 weeks <br />4.204525e-4 months <br />.

Extra efforts were made to return the MDFP to service within the seven day period. Work activities were scheduled 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> per day, seven days a week.

In fact, the necessary work _was completed, and the intake flooded within the seven day time frame; however, a number of administrative actions (remove

clearances, post maintenance testing, return MDFP to service, etc.)

remained. Through misunderstanding and a resultant oversite, these actions

, were not completed in the expeditious manner needed to meet the seven day requirement . Once recognized, actions were taken, and the MDFP was returned to service, although 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br /> and 50 minutes beyond the seven day period.

The Engine Driven Fire Pump was operable the entire time the Motor Driven Fire Pump was out-of service. The Motor Driven Fire Pump could have been quickly made available for use in the event of an emergency until the afternoon of February 6,1986, when the CW intake bay was dewatered. Personnel involved in I

the above oversite and their supervision will review this event to emphasize the importance of closer attention to returning this type equipment to service within specified time periods.
