Subject: Lost Iridium-192 Seed

The Region V Duty Officer was notified by telephone at 4:30 P.M. on fiarch 14, 1980
by the Providence Hospital Radiation Safety Officer that a 0.6 millicurie iridium-192
seed was missing from one of the ribbons when an interstitial implant was removed.
The licensee stated that radiological surveys have been conducted in the room and on
the patient and the missing seed cannot be found. Regional followup Will be made to
determine the current status of the iridium-192 seed. '
The licensee does not plan a news release. '
