- 1. The interview took place at approximately 3:10 PM and lasted vrt:n enmimately i 5:00 PM on April 13,1979 The Interview was held at Mama's Pirra thse ,
in Cleburne, Texas.

Mr. Tomlinsen cf our office, and another party who accompenled I had to promise the party who accompanied that his identity vcmid not leave the place of interview. I felt that the promise was justified on the basis that he appeared to have Information which corraborates allegatio'ns. The party remains in the empkryt e W a,4. P. I Results of interview: I ,
- 1. Summary of Allegation: ._
ade the allegation to Mr. Iluhacek that during a concrete pour in January 1979 on the Unit 1 dome that concrete was washed away and was replaced'by a grout. retterated in the Interview that the event , did occur / which was verified by the unidentifled party. The, unidentified party however stated t, hat gravel was in the concrete so thatkas hot grout.

I interviewed the D & lt QC Inspector of record, one Ray Drown, who described the-situation, 11 e indicated that the pour had gone throughout the dey as a planned very slow rate using buckets. As the day developed the weather , ,, turned mii'ty with occasional light rain. During the evening she weather worsened and finally became a fulllledged rain stormwith thunder and
,, lightening. Dubssposure of personnel to the hazards of lightening , the pour was stopped at eight , the pour area [c'o'vered up to prevent the logress of rain and the crews sent home, which inclusted the D a R inspectors it now appears that some work went on up on the dome well after 8:00 Pm and that some concrete was delivered to the after the batch plans was closeddo_ repair a washoW Proposed Course of Action This allegation is of a type that cannot be verified or refmed by any type of records review. QC had already gone home and the batch plant appears to have been closed. In his original alleCation to Mr. Ilubacek, aid that the identity of a lot of Q nni non-Q piping sir inch or les, was being lost in sand blasting prior to painting, stated during the interview Jhat pljong referred to was nel fabricated speel pieces but was bulk pipe sections usually known ' as Joints. lie stated ti at on a number of onwslenn he had been dispatched Ip take bulk pipe to the gwilnt shop and nrin pich it up from the shop and deliver it elsewherer- lie had pickert up the identification peoblem during conversations with the paint whop peopic. The problem according to and the other party extends to any material routinely blasted and gwinted . This wouhl then include hanger and embed materials.

- 3. Summary of Allegation:
In his original allegation, stated that a steam pipe intended for the unit one turbine fell off a truck nd struck a railrrud track, it was taken back to the storage area and hidden. tated in the interview tint the . pipe was originally stored at so-called D 'd spur which is the non-Q , storoEc area for the turbine equipment. Ile Indicated the p' ipe was oringinally loaded on a truck and fell off. cre dinpatched to the area where they reloaded it on the truck and it was taken back into the storage yard, it was intended ,that the second shlit people would appear to have done no work on the spool no that the blame for the incident would j fall on othenif any actual damage had been done. stnted that he was not aware of whether the spool had been damaged or not. lie stated that he was ! aware that the spool was non-Q but thought we should know about it, since it was typical of the way Q and non-Q material was being handled when no one was around. Jones ,. personnel. s- Assessment of Allegation: - The NRC staff independently reviewed 20 of 28 . l concrete basemat placement packages. The staff review of the 20 specifically sampled the first three blocks (blocks 1, 2, and 6)placed, placements since l these placements preceded the Stop Work Order which was issued to achieve corrective action of improper placement activities. . several violations of specifications and procedures during this review, including:
o Out-of-tolerance concrete test results (air content, slump, mixing time, mixer revolutions, volume of water added), o concrete testing frequency, o vibrating (concrete and reinforcing steel), o excessive movement of concrete,
. .o excessive lift height, and o concrete curing documentation.
The NRC staff review of J. A. Jones records of the basemat placement packages showed that these violations were identified by EBASCO Quality Assurance (QA) $ in various placement packages. When a violation was identified, imediate corrective action was taken in the "feldi when possible. A discrepancy notice (DN), and when necessary a nonconformance report (NCR), was written to document , the violation and'to establish appropriate corrective action. If the violation l involved a change in the original design specification, a field change request (FCR) or a design change notice (DCN) was written to address the violation. For further discussion on the above violations, refer to Allegatlons 139/140. Although specifications andiprocedures were violated during concrete placements, each violation was identified and documented by EBASCO QA, and corrective actions were tak4n.

In view of this information, the alleger's concern was whether the other 5% of the field verifi- ~ cations,had been performed prior to closure of the NCR.
.( : -.[O The NRC staff contacted the former Mercury employee in order to determine which hanger supports had not been reviewed. He stated that all but eight of the supports requiring rework had been completed, inspected, and documented.
The NRC staff reviewed EBASCO NCR W3-2333 and found these eight supports documented in the closed Mr. package. The staff reviewed the documentation and the v. .ainder of NCR W3-2333 for adequate closure. The staff found that the NCR adequately addressed field verification of hanger supports and had been dispositioned and closed out properly. This allegation has neither safety significance nor generic implications.
