DPR-4 Docket No. 50-146 Response to the August 18, 1995 Request for Additional Information Reporting Completion of Corrective Actions The purpose of this letter is to respond to your Request for Additional l Information dated August 18, 1995 regarding the inadvertent breach of the SNEC 1 facility Containment Vessel liner during core bore operations in support of characterization. It was requested that we inform you of the completion of the delineated corrective actions associated with item I which were not identified as being complete in our letter of July 27, 1995.

Item 1, Part 3 stated that "Information will be disseminated on the need to field verify Saxton drawings before use." Personnel involved in all aspects of the characterization core boring task were either part of the Human Performance Enhancement System (HPES) review of the event or were required to read the resultant report. This item is co:aplete.

Item 1, Part 7 stated "The event and the ' lessons learned' will be reviewed as a topic on an upcoming session of Engineering Support Personnel (ESP)

Training." A training session was initiated and presented to accomplish two objectives: 1) increase responsible personnel's awareness of the need to adequately verify the correctness of drawings prior to use and 2) impress upon the individuals their responsibilities as reviewers. This training module has been completed for SNEC and appropriate GPU Nuclear personnel and the item is complete. A copy of the training handout material is included as Attachment 1.

Item 1, Part 8 stated "The Saxton procedure process was reviewed and is being revised. The revision will improve the review process and more clearly define reviewer responsibilities and assignments." Procedure 6575-ADM-4500.07 the "SNEC Procedure Development, Change Requests and Safety Review" procedure was revised, approved and issued on November 14, 1995. The item is complete. A copy of the procedure is included as Atte.chment 2.

Finally, the permanent plugging of the bored hole was completed on February 6, 1996. Two applications of a polyurethane hydrophilic foam grout material were made to eliminate the ground water in-leakage. Once the in-leakage was verified stopped, the hole was filled to the level of the surrounding concrete with hydraulic cement.
