LFR CONTINUATION -- RO# 2-79-029 Facility: BSEP Unit No. 2 Eveat Date: 6-2-79 PT 19.6.0.1, during the 1979 refueling outage. The snubber was to have been installed by contract auintenance personnel who were shown the proper location for reinstallation. They failed to install the snubber. Management personnel of that contract maintenance company have been informed of the seriousness of this job omission. This has also been reviewed by plent maintenance fereman and will be reviewed by all mechanics. Testing of snt.bbers will contum _ per Technical Specifications. Maintenance procedures are being revised suct that when a snubber is removed for mainterance, it will be verified reinstalled and operable.
