Dear Mr. Seyfrit:

FILE NO. G9.5.2 OTHER CORRESPONDENCE RIVER BEND STATION - UNIT I The placement of safety related (Category I) concrete was resumed at the River Bend Station on December 18, 1979. As we have discussed with your regional office, and as you observed, there were inefficiencies in this initial placement. Initially supervision was below standard and organization suffered from deficiences in planning. It should be noted, however, that no Quality Program deficiencies or specification nonconformances were observed prior to or during the placement.

The purpose of this letter is to assure you that we have taken steps to insure that future Category I concrete placements are performed in a highly efficient manner. The following major changes have been made:

1. The chief supervisor was replaced with a more experienced individual within 45 minutes of the first observed difficulties. Both concrete con-struction teams now have two levels of supervisors with prior nuclear construction experience.
2. Commencing immediately a pre-placement concrete planning session will be conducted prior to each concrete placement.

Construction supervisors down to the foreman level, Field Quality Control inspectors and Quality Assurance personnel will attend. This coordinating activity is for the purpose of preventing difficulties and for improving the efficiency of concrete placement. For the next three months this 8002150 b) '2

Seyfrit Page 2 RBG 7058 December 21, 1979 planning effort will be in effect for all categories of concrete placements, but will eventually be reduced to only major safety related placements after it has been determined by the GSU Quality Assurance organization that an acceptable level of proficiency has been achieved and maintained.

3. Training sessions will be conducted for difficult placements. Engineering and Quality Assurance will be involved in this training.
