Dear Mr. O'Reilly:

At 1500 on December 22, 1980, the stack sample pump was found not running during a routine filter change. An investigation of strip charts for the stack off-gas monitor and the stack sample pump time-running integrator revealed that the pump had stopped at approximately 1800 on December 17,~

1980. A thorough review of strip charts and plant parameters for systems that exhaust through the stack from both units determined that no significant release was made during the time period when this monitor was inoperable.

An investigation of the stack sample pump determined that the main power fuse for the pump was blown. The system's electrical circuits were-examined and no problems could be detected. The blown fuse was replaced and the system returned to normal operation with no further problems noted.

The pump low flow alarm which annunciates on both Unit Nos. 1 and 2 control panels did not annunciate indicating a low flow condition during.,

this event. The flow switches which activate these alarms were investigated and found to be out of adjustment. These switches were adjusted and tested to alarm at their correct set points.
