1.7.1ORGANIZATIONI.7.1.ISCOPEAmericanElectricPowerServiceCorporation(AEPSC)isresponsibleforestablishingandimplementing=-thegualityAssuranceProgramfortheoperationalphaseoftheOonaldC.CookNuclearPlant(CookNuclearPlant).Althoughauthorityfordevelopmentandexecutionofvariousportionsoftheprogrammaybedelegatedtoothers,suchascontractors,agentsorconsultants',AEPSCretainsoverallresponsibility.AEPSCshallevaluateworkdelegatedtosuchorganizations.Evaluationsshallbebasedonthestatusofsafetyimportanceoftheactivitybeingperformedandshallbeinitiatedearlyenoughtoassureeffectivequalityassuranceduringtheperformanceofthedelegatedactivity.ThissectionofthegualityAssuranceProgramDescription(gAPD)identifiestheAEPSCorganizationalresponsibilitiesforactivitiesaffectingthequalityofsafety-relatednuclearpowerplantstructures,systems,andcomponents,anddescribestheauthorityanddutiesassignedtothem.ItaddressesresponsibilitiesforbothattainingqualityobjectivesandforthefunctionsofestablishingthegualityAssuranceProgram,andverifyingthatactivitiesaffectingthequalityofsafety-relateditemsareperformedinaccordancewithgAProgramrequirements.1.7.1.2IMPLEMENTATION1.7.1.2.1SourceofAuthoritTheChairmanoftheBoardandChiefExecutiveOfficerofAmericanElectricPowerCompany,Inc.(AEP)andAEPSCisresponsibleforsafeoperationoftheCookNuclearPlant.AuthorityandresponsibilityforeffectivelyimplementingthegAProgramforplantmodifications,opera-tionsandmaintenancearedelegatedthroughtheAEPSCSeniorExecutiveVicePresident-EngineeringandConstruction,totheAEPSCVicePresident-NuclearOperations(ManagerofNuclearOperations).1.7-4July,1989 Intheoperationofanuclearpowerplantthelicenseeisrequiredtoestablishclearanddirectlinesofresponsibility,authorityandaccoun-tability.Thisrequirementisapplicabletotheorganizationprovidingsupport.to-theplant,aswellastotheplantstaff.TheAEPSCcorporatesupportoftheCookNuclearPlantistheresponsibilityoftheentireorganizationunderthedirectionoftheManagerofNuclearOperationswhomaintainsprimaryresponsibilityfortheCookNuclearPlantwithinthecorporateorganization.TheAEPSCVicePresident-NuclearOperationsistheManagerofNuclearOperations.AllotherAEPSCdivisionsanddepartments,otherthanthegualityAssuranceDivision,havingasupportingrolefortheCookNuclearPlantarefunctionallyresponsibletotheManagerofNuclearOperations(referenceFigure1.7-1).Inordertofacilitateamorethoroughunderstandingofthesupportfunctions,someoftheresponsibilities,authorities,andaccountabilitieswithintheorganizationareasfollows:I)TheresponsibilitiesoftheManagerofNuclearOperationsshallbededicatedtotheareaofCookNuclearPlantoperationsandsupport.2)TheManagerofNuclearOperationsshallberesponsiblefor,andhastheauthoritytodirectallCookNuclearPlantoperationalandsupportmatterswithinthecorporationandshallmakeorconcurinallfinaldecisionsregardingsignificantruclearsafetymatters.3)AEPSCdivisionanddepartmentmanagersresponsibleforCookNuclearPlantmattersshallbefamiliarwithactivitieswithintheirscopeofresponsibilitythataffectplantsafetyandreliability.TheyshallbecognizantofandsensitivetointernalandexternalfactorsthatmightaffecttheoperationsofCookNuclearPlant.4)AEPSCdivisionanddepartmentmanagersresponsibleforCookNuclearPlantmattershaveacommitmenttoseekandidentifyproblemareas1.7-5July,1989

't>$t ThePlantManager,underthedirectionoftheAEPSCVicePresident-NuclearOperations,isresponsibleforestablishingCookNuclear'PlantgualityControlandimplementingthegualityAssuranceProgramattheCookNuclearPlant.Management/supervisorypersonnelreceivefunctionaltrainingtothelevelnecessarytoplan,coordinate,andadministratethoseday-to-dayverifi-cationactivitiesofthegAProgramforwhichtheyareresponsible.AEPSChasanindependentoff-siteNuclearSafetyandDesignReviewCommittee(NSDRC)whichhasbeenestablishedpursuanttotherequirementsoftheTechnicalSpecificationsfortheCookNuclearPlant.ThefunctionoftheNSDRCistooverseetheengineering,design,operation,andmaintenanceoftheCookNuclearPlantbyperformingauditsandindependentreviewsofactivitieswhicharespecifiedintheFacilityOperatingLicenses.TheCookNuclearPlanton-sitereviewgroupistheIndianaMichiganPowerCompany(IBM)PlantNuclearSafetyReviewCommittee(PNSRC).ThiscommitteehasalsobeenestablishedpursuanttotherequirementsoftheCookNuclearPlantTechnicalSpecifications.ThefunctionofthePNSRCistoreviewplantoperationsonacontinuingbasisandadvisethePlantManageronmattersrelatedtonuclearsafety.1.7.1.2.3CororateOranizationAmericanElectricPowerComanAEP,theparentholdingcompany,whollyownsthecommonstockofallAEPSystemsubsidiary(operating)companies.ThemajoroperatingcompaniesandgenerationsubsidiariesareshowninFigure1.7-2.TheChairmanoftheBoardofAEPistheChairmanoftheBoardofAEPSCandistheChiefExecutiveOfficerofAEPSCandalloperatingcompanies.Theresponsibility1.7-7July,1989 Jgl)!llA forthefunctionalmanagementofthemajoroperatingcompaniesisvestedinthePresidentofeachoperatingcompanyreportingtotheAEPSCPresidentandChiefOperatingOfficerwhoreportstotheAEPSCChairmanoftheBoard.AmericanElectricPowerServiceCororationTheresponsibilityforadministrativeandtechnicaldirectionoftheAEPSystemanditsfacilitiesisdelegatedtotheAmericanElectricPowerServiceCorporation(AEPSC).AEPSCprovidesmanagementandtechnologicalservicestothevariousAEPSystemcompanies.0eratinComaniesTheoperatingfacilitiesoftheAEPSystemareownedandoperatedbytherespectiveoperatingcompanies.Theresponsibilityforexecutingtheengineering,design,construction,specializedtechnicaltraining,andcertainoperationssupervisionisvestedinAEPSCwhileallorpartoftheadministrativefunctionalresponsibilityisassignedtotheoperatingcompanies.InthecaseofCookNuclearPlant,IBMgeneralofficestaff(headquarters)providespublicaffairs,accounting,industrialsafetydirectionandprocurementsupport.TheCookNuclearPlantisownedandoperatedbyISHwhichispartoftheAEPSystem.1.7.1.2.4ualitAssuranceResonsibilitofAEPSC1)AEPSCprovidesthetechnicaldirectionoftheCookNuclearPlant,andassuchmakesthefinaldecisionspertinenttosafety-relatedchangesinplantdesign.Further,AEPSCreviewsNuclearRegulatoryCommission(NRC)letters,bulletins,notices,etc.,forimpactonplantdesign,andtheneedfordesignchangesormodifications..July,1989 2)AEPSCfurnishesqualityassurance,engineering,design,construction,licensing,NRCcorrespondence,fuelmanagementandradiologicalsupportactivities.3)AEPSCprovidesadditionalserviceinmatterssuchassupplierqualification,procurementoforiginalequipmentandreplacementpartsandthe.processofdedicatingcommercialgradeitemsorservicestosafetyrelatedapplications.4)TheAEPSCgADivisionprovidestechnicaldirectioninqualityassurancematterstoAEPSCandtheCookNuclearPlant,andoverseestheadequacyandimplementationofthegAProgramthroughreview,andauditactivities.5)CognizantEngineer-(e.g.,SystemEngineer,EquipmentEngineer,LeadEngineer,ResponsibleEngineer,etc.)-Thecognizantengineer,and/orengineerwiththeothertitlesnoted,isthatAEPSCindividualwhoprovidestheengineer/designexpertiseforaparticularareaofresponsibility.Thisresponsibilityincludestheimplementationofthequalityassuranceandqualitycontrolmeasuresforsystems,equipment,structures,orfunctionalareasincludedinthatindividual'sresponsibility.Thevarioustitlesusedfortheidentificationofanindividual'sresponsibilityandassignmentshallbeunderstoodtomeanthesameascognizantengineerintherespectiveareasofresponsibility.ualitAssuranceResonsibilitofISM-CookNuclearPlantIBM'splantstaffoperatestheCookNuclearPlantinaccordancewithlicensingrequirements,includingtheTechnicalSpecificationsandsuchothercommitmentsasestablishedbytheoperatinglicenses.ThePlantManagerInstruction(PMI)systemandsubtierinstructionsandproceduresdescribethemeansbywhichcomplianceisachievedandresponsibilitiesareassigned,includinginterfaceswithAEPSC.Figure1.7-3indicatestheorganizationalrelationshipswithintheAEPSystempertainingtotheoperationandsupportoftheCookNuclearPlant.July,1989'
