22. A copy of your most recent Top Ten List (equipment reliability and/or management focus). Include associated CR, action plan, status, and brief history (when did issue originate).

23. Copy of all root cause evaluations (assuming that that is the top level root cause review and that this would not be a huge population) and a listing of all apparent cause evaluations completed since March 1, 2006.
