: 1. PSE&G has instituted a site photo identification requirement for all employees and contractors. Individuals are required to present photo identification for Fitness-for-Duty (FFD) testing, access training and processing. There was no site photo identification requirement previously and many individuals arrived at the collection facility without photo identification. As a result, the individual's supervisor was called for identification. This situation resulted in confusion and unnecessary delays. The new requirement was a result of our experience during the initial stages of program implementation.

In addition to the problem with cough suppressants and analgesics, wide-spread consumption of poppy seed containing food and the common knowledge that poppy seeds may result in a positive drug test result, makes it almost impossible to declare a positive per the rule. A significant amount of expense can be eliminated by removing opiates from the panel of drugs tested in areas of the country and/or states where heroin abuse does not appear to be common.

After several unsuccessful attempts to reach the Screening Supervisor by telephone, she hand-carried the revocation paperwork to the Screening Supervisor's office. The Screening Supervisor immediately notified Security to terminate the individual's badge and was informed that the individual had re-entered the protected area. PSE&G subsequently determined that the individual returned to his work location because the EAP Counselor was off-site that day and thus unavailable to conduct/schedule the interview. Individuals must comply with EAP recommendations to be eligible for future unescorted access.

SUMMARY

OF MANAGEMENT ACTIONS INITIATIVES TAKEN

1. PSE&G has instituted a site photo identification requirement for all employees and contractors. Individuals are required to present photo identification for Fitness-for-Duty (FFD) testing, access training and processing. There was no site photo identification requirement previously and many individuals arrived at the collection facility without photo identification. As a result, the individual's supervisor was called for identification. This situation resulted in confusion and unnecessary delays. The new requirement was a result of our experience during the initial stages of program implementation.
2. Referral to an Employee Assistance Program (EAP) Counselor is required for all employees and contractors determined to be positive. This referral provides an opportunity for substance abuse evaluation and/or assessment for addictive potential. Those individuals who do comply with our EAP requirements are potential candidates for reinstatement of unescorted access. Those individuals who do not wish to comply with our EAP requirements waive their opportunity for regaining unescorted access at PSE&G's nuclear facilities.

We believe that our EAP program has been very successful.

3. PSE&G developed an alcohol policy that exceeds the rule requirements. Any individual determined to have had a blood alcohol level of greater than or equal to 0.04% BAC, at any time during their workday, is considered positive and subject to the same sanctions as individuals testing greater than or equal to 0.04% BAC. One (1) individual has been declared a positive under this policy for the period ending June 30, 1990.
4. PSE&G found that supervisors need additional guidance on the support available from Administration, Medical, Employee Relations and Industrial Relations in implementing the FFD Program. Supervisors need additional guidance in making job assignment decisions for individuals whose performance may be affected by illness, fatigue, stress, prescribed medications, and over-the-counter medications. To address these issues, PSE&G developed a video and added a module to the supervisory FFD training. The training module enhances supervisory awareness and provides guidance on PSE&G's policy as it relates to FFD Program implementation and supervisory responsibility. PSE&G has taken a proactive approach to FFD beyond mere drug and alcohol testing.

LESSONS LEARNED

1. PSE&G recommends that the NRC consider removing opiates from the panel of drugs to be tested. We have found that testing for opiates significantly delays preaccess processing, and significantly undermines the program acceptance and credibility. M-A-M is only present for a very short period of time, and there is wide-spread use of opiate cough suppressants and analgesics. The present requirement that demands expensive GC/MS confirmation to supposedly "rule out heroin abuse" is extremely expensive due to the type of testing required for detection. In the five (5) years of testing by PSE&G at its nuclear facilities, there have been no detected cases of heroin abuse. In addition to the problem with cough suppressants and analgesics, wide-spread consumption of poppy seed containing food and the common knowledge that poppy seeds may result in a positive drug test result, makes it almost impossible to declare a positive per the rule. A significant amount of expense can be eliminated by removing opiates from the panel of drugs tested in areas of the country and/or states where heroin abuse does not appear to be common.
2. PSE&G strongly believes that a FFD Program cannot be functionally practiced as only a drug and alcohol detection/deterrence program. The level of decision making involves more than just review of drug and alcohol results.

Medical Review Officer (MRO) involvement is essential and critical to a properly functioning FFD Program. PSE&G mentions this since the DOT is considering the removal of the MRO review requirement for all test results.

EFFECTIVENESS EVALUATIONS

1. PSE&G has undergone two (2) thorough audits of its FFD Program; one by an internal auditing group and the other by the NRC. PSE&G's Medical Department periodically conducts self-assessment audits. Audit results to date have detected no significant problems and confirmed PSE&G's commitment to a premiere program.
2. PSE&G conducts informal trending (i.e., recognizing the expense of testing for opiates, popularity of certain drugs, population resulting in the most positives, client availability for testing, etc.) to obtain feedback on program effectiveness. The NRC is emphasizing the need for trending mechanisms, as evidenced by our recently completed audit. Since the rule is vague on this matter, PSE&G has requested that the NRC provide licensees with additional guidance. Sample reports containing FFD testing results are attached for your review. PSE&G is presently developing additional reports to assist the trending effort.

SUMMARY

OF EVENTS REPORTED BY TELEPHONE

1. PSE&G reported an incident on March 5, 1990, involving the first "random" drug test determined to be positive. The tested drug was cocaine. The individual was a long-term contractor laborer. In accordance with PSE&G policy, the individual was referred to EAP for assessment and evaluation. Individuals must comply with EAP recommendations to be eligible for future unescorted access.

summary of Incident The incident was reportable because the individual's unescorted access was not terminated quickly enough to prevent re-entry into the protected area. The duty nurse escorted the individual to the Psychological Services Department (across the hall from the Medical Department) to schedule a followup interview with an Employee Assistance Counselor. The nurse then returned to her work area to notify the Screening Supervisor to terminate this individual's badge. After several unsuccessful attempts to reach the Screening Supervisor by telephone, she hand-carried the revocation paperwork to the Screening Supervisor's office. The Screening Supervisor immediately notified Security to terminate the individual's badge and was informed that the individual had re-entered the protected area. PSE&G subsequently determined that the individual returned to his work location because the EAP Counselor was off-site that day and thus unavailable to conduct/schedule the interview. Approximately one (1) elapsed from determination of a positive drug test to termination of unescorted access. A number of factors contributed to this incident. PSE&G conducted a fact-finding investigation that resulted in appropriate corrective actions. The corrective actions involved counselling the nurse and revising the associated procedure.

2. PSE&G reported an incident on May 23, 1990, involving a supervisor testing positive. A breathalizer test yielded an alcohol level above the 0.04% BAC limit during the individual's workday. This individual is a permanent licensee employee in the position of Project Manager. In accordance with PSE&G policy, the individual was referred to EAP for assessment and evaluation. Individuals must comply with EAP recommendations to be eligible for future unescorted access.

Summary of Incident This incident was reported, as required by the FFD Rule, because it involved a supervisor found to be positive.
