ASSESSMENT FOLLOW-UP LETTER FOR ARKANSAS NUCLEAR ONE, UNITS 1 AND 2

Dear Mr. Browning:

As a result of our continuous review of plant performance, the U. S. Nuclear Regulatory Commission (NRC) updated its assessment of Arkansas Nuclear One, Units 1 and 2. The NRCs evaluation consisted of a review of performance indicators and inspection results. This letter informs you of the NRCs assessment and plans for future inspections at your facility. This letter supplements, but does not supersede, the annual assessment letter issued on March 4, 2014.

In a letter to you dated June 23, 2014, the NRC informed you of its final risk significance determination associated with inspection findings in the Initiating Events Cornerstone for Unit 1 and Unit 2. Those findings, which we concluded were of substantial safety significance (Yellow),

involved your staffs failure to provide adequate oversight for the planned movement of the Unit 1 turbine stator in March 2013 using a temporary hoisting assembly. More specifically, your staff failed to identify deficiencies in a vendor calculation and an incorrectly sized component in the north tower structure of the temporary hoisting assembly. In addition, the temporary hoisting assembly was not designed for at least 125 percent of the projected hook load, and a load test was not performed in all configurations for which the temporary hoisting assembly would be used.

Also, on June 18, 2014, the NRC informed you of our final significance determination with respect to a Greater-than-Green inspection finding in the Security Cornerstone.

Given the significance of the findings referenced above, the NRC determined the performance at Arkansas Nuclear One, Unit 1 and Unit 2, to be in the Degraded Cornerstone Column of the Reactor Oversight Process Action Matrix beginning the first quarter of 2014. The NRC plans to conduct a supplemental inspection to review the actions taken to address the performance issues associated with the Yellow findings in accordance with Inspection Procedure 95002, Supplemental Inspection for One Degraded Cornerstone or Any Three White Inputs in a Strategic Performance Area. The NRC will plan this inspection activity when you provide written notification of your readiness for the inspection.

Browning This supplemental inspection will provide assurance that the root and contributing causes of risk-significant performance issues are understood, that the extent of condition and the extent of cause of risk-significant performance issues are identified, and that corrective actions for risk-significant performance issues are sufficient to address the root and contributing causes and prevent recurrence. This inspection will also determine if safety culture components caused or significantly contributed to the risk-significant performance issues.

The NRC also plans to conduct a supplemental inspection to review the actions taken to address the performance issues associated with the finding in the Security Cornerstone. The NRC will plan this inspection activity when you provide written notification of your readiness for the inspection.
