I. Introduction aad Summary The South Texas Project Electric Generating Station (STPEGS) is located on 12,300 acres in Matagorda County, Texas, approximately 15 miles southwest of Bay City along the west bank of the          lorado River. STPEGS censists of two 1250-MWe units, a 7,000 acre cooling reservoir, a cooling pond, attendant pumping and discharge facilities. There are approximately 305 miles of transmission line corridors originating from the STPEGS site.         STPEGS is jointly owned        by Houston Lighting & Power Company (HL&P), Central Power & Light Company, the City of Austin, and the City of San Antonio. HL&P has been designated as Project Manager for the owners and is responsible for implementation of all environmental programs.
The following report fulfills requirements established in Sections 3.1 and 5.4.1 of Appendix B to Facility Operating License Nos. NPF-76 and NPF-80 South Texas Project Units 1 and 2 (Environmental Protection Plan (Nonradiological)), March 1989. The time period for this report covers January 1, 1988 through December 31, 1988 inclusive, as required by Section 5.4.1 of Appendix B of the aforementioned licenses.
Environmental monitoring for STPEGS during 1988 was conducted by plant and corporate (HL&P) personnel in acco.rdance with federal and state regulations and applicable plant procedures.
The following report offers a compendium of plant environmental activities and notable events for 1988. With the exceptions noted in Sections V and VI of this report, STPEGS was in compliance with all nonradiological, environmental-related federal, state, and local laws and regulations, license-related documer.ts and commitments, and plant procedures.
II. Fog Monitoring Program Phase one of the fog monitoring program as described in Section 6.2.4.2 of Amendment 10 to the Environmental Report began in May 1987 and collteted one year of data.         The second phase will begin following startup of Unit 2 and will continue for one I                                            year. A report on the results of the program will be submitted l                                            in a timely manner following completion of the monitoring.
III. Aquatic Monitoring Program No aquatic monitoring was required by the U.S. Environmental Protection Agency (EPA) or the state of Texas under the authority of the Clean Water Act for this time period.

IV. Plant Operations Any plant design, plant operation, test or experiment which may involve a change in the Environmental Protection Plan (EPP) or which could have a significant environmental impact is. required l                     to undergo a nonradiological environmental evaluat3on as per l                     STPEGS Site procedures OPGP03-ZA-0003 (License Compliance Review) and OPGP03-ZA-0017 (Non-Radiological Environmental

environmental impact, in the nonradiological area, resulting from operational activities. Leviews of applicable plant designs, operations, tests and experiments are discussed below.

B. Main Cooling Reservoir Inspections of'the Main Cooling Reservoir (MCR) embankment were conducted by Harza Engineering Company. Inspection findings are documented by Harza via transmittal in a weekly inspection report to Houston Lighting & Porar.              A summary of significant findings is discussed below.
Pumping operations from the Colorado ~ River to the MCR were begun March 8, 1988, and continued until April 8, 1988. Pumping resumed again from June 4 until June 9 of 1988. Dredging operations were conducted by the HL&P contractor in the River Hakeup Pumping Facility (RMPF) and by the Corps of Engineers' contractor between the Rh/F and the Spillway Discharge Channel.
Spoil from these operations was placed in the designated spoil area.        Remedial                   ,

C. Environmental Permit Monitoring Monitoring for Site compliance with applicable federal and state environmental permits (Texas' Air Control Board (TACB)
Permit No. R-7410, EPA Permit No. PSD-TX-209, TWC Permit No. 01908, and EPA Permit No. TX0064947) was conducted by plant personnel in accordance with Site procedures Site Instruction 3.10 (Environmental Protection Control Program) and CPCP10-ZH-0004 (Site-Environmental Monitoring). The Texas Air Control Board conducted an air quality / auxiliary boiler permit compliance inspection on June 28, 1988. With those exceptions noted earlier in this section and in Section VI of this' report, STPEGS was in compliance with applicable federal and state environmental regulations including the aforementioned permits.
EPA Permit No. TX0064947.was renewed, amended, and re-issued with an effective date of August 16, 1988.
Significant changes from the previous permit included the lowering of the reservoir blowdown elevation limitation to 38 ft. MSL, the addition of biomonitoring requirements on Outfall 001 (Reservoir Blowdown), and the imposition of a fecal. coliform limitation on Outfall 002 (Construction Sanitary Waste Treatment System)._ (Attachment E)
Compliance with TWC Permit No. 01908 and EPA Permit No.
l;         TX0064947 is documented via Self Reporting Data Sheets as I          per OPCP10-ZH-0004 (Site Environmental Monitoring). This information is then transmitted to the TWC. Groundwater monitoring was conducted in accordance with TWC Permit No.
01908 and results for 1988 are attached to this report.
(Attachment C) The Lower Colorado River Authority (LCRA) conducted monthly inspections of wastewater treatment
(          facility compliance with TWC permit limits. Wastewater L          permit compliance inspections were also' conducted by the EPA and the TWC on May 3-5, 1988, and July 14, 1988 respectively. Inspection findings are documented by the LCRA via monthly inspection reports to Houston Lighting &
Power and via memorandum by thc EPA and the TWC. A potable water system inpection war conducted December 20, 1988, by the Texas Department of Health and documented via memoranda to Houston Lighting & Power.

i VI. Environmental Protection Plan Noncompliance                       I f
i On August 31, 1987, the U.S. Environmental Protection Agency      I issued Administrative Order Docket No. VI-87-278 NPDES Permit      l No. TX0064947 (South Texas Project) against the South Texas        l Project Electric Generating Station for alleged permit exceedances in late 1986 and early 1987. Due to significant      '
improvements in the reduction of permit parameter exceedances in 1988, Houston Lighting & Power has requested that this Administrative Order be closed. (Attachment A) l For the time frame of this report (1988), thirty-three (33) environmental permit violations were incurred by STPEGS.
(Attachment D) All of these violations involved exceeding effluent or emission limits established in TWC Permit No.         !
01908, EPA Permit No. TX0064947, or U.S. EPA Permit No.
PSD-TX-209. In additior. to the aforementioned permit violations, one reportable oil spill occurred in 1988. Below is a detailed review of the deficiencies that occurred during 1988 and description of corrective actions taken to prevent the I
recurrence of the deficiencies indicated.
A. Outfall 002 (Construction Sanitary Waste Treatment System)
During 1988, there were six (6) violations associated with Outfall 002. The TSS daily average was exceeded in February when the samples for that month exceeded or were near the permitted daily average although maximum daily limits were not exceeded. On July 26, 1988, the TSS daily maximum was exceeded due to hydraulic overload of the system. This also resulted in the TSS daily average being exceeded for the month of July. High settleabilities and higher than normal surge flows resulted in the BOD 5 maximum limit being exceeded on August 4, 1988.
This also resulted in the BOD 5 daily average being exceeded for the month of August. On September 6, 1988, the fecal coliform maximum limit was exceeded when higher than normal settleable solids level caused sludge carryover and resulted in fecal co11 form levels which were not effectively treated with the chlorine injection on that day.      The system was returned to complian operations after settleabilities were decreased.
This treatment system has experienced occasional hydraulic overload conditions in 1988. An                  ,
aerated surge basin was placed in operation in            i October to equalize inflows, provide supplemental treatment capability, and reduce

the potential for hydraulic overload conditions.
No further violations have been incurred on this outfall since the installation of the surge basins.
B. Outfall 101 (Neutralization Basin)                          i l
l Twelve (12) violations were incurred on Outfall             i 101 in 1988. Ten (10) of the violations were                 l bypasses of the system which resulted from i
overflows which were not recovered for proper                '
processing through the system. On January 29, 1988 the Unit 1 Auxiliary Feedwater Storage Tank overflowed and approximately 300 gallons of process water entered the storm drainage system.
On February 21, 1988, approximately 30 gallons of regenerant wastewater overflowed the Equalization Basin to the storm drainage system due to a malfunctioning valve which was subsequently repaired. On February 25, 1988, approximately 100 gallons of process water entered the storm drainage system when a temporary hose on the Auxiliary Boiler pad became disconnected. On March 20, 1988, approximately 500 gallons of water entered the storm drain system from various Auxiliary Boiler leaks. On April 13, 1988, an overflow of                     l approximately 300 gallons entered the storm drain system from the Unit 2 Secondary Makeup Storage Tank which was inadvertently overfilled.
On April 19, 1988, 2,000 gallons of chemically treated water was discharged to the storm drain from the Unit 2 Refueling Water Sterage Tank during flushing activities associated with construction due to personnel error. On April 23, 1988, 1,000 gallons of chemically treated water was released to the storm drain from an unsecured feedwater flush line flange on Unit 2.
due to leaking remote operated valves. On July 13, 1988, 2,000 gallons of chemically treated water was released to the storm drain from a failed flange on the Unit 2 CP sump discharge line. On August 10, 1988, the Inorganics Basin overflowed to the storm drain system when the transfer pumps malfunctioned. On August 19, 1988, an overflow of approximately 2,000 gallons from the Unit 1 Secondary Makeup Water Storage Tank occurred when a valve was inadvertently left partially open.
Upon discovery of the aforementioned occasions, immediate actions were taken to minimize the spill or overflow and prevent any further input

to the storm drainage system. Long term corrective actions included additional instruction for plant personnel concerning permit compliance requirements, equipment repairs, initiation of a hose control program, and installation of supplemental containment barriers.        In addition, development and installation of a permanent containment system is in progress.
In addition to the ten (10) overflows / bypasses              _j of outfall 101 previously discussed, two (2)                      '
                                                        -other violations were incurred on outfall 101 in 1988. A frequency violation occured on July 12, 1988, when a TSS sample sent for analysis from the Site was lost in transit to the contract laboratory. Administrative changes were instituted to improve accountability of saeples sent offsite for analysis. A pH maximum was exceeded on August 10, 1988, as a result of a procedural inadequacy. Administrative guidelines were proceduralized to prevent further occurrences.
C. Outfall 201 (Oily Waste Treatment System)
Eight (8) violations were incurred on outfall 201 in 1988. Four oil and grease _ maximums were exceeded on April 4, April 11, April 19, and l                                                        July 17, 1988, due to mechanical deficiencies i                                                        and system design inadequacies. In addition, the high oil and grease values in April of 1988 resulted in the daily oil and grease average limit being exceaded in that month. Also, from November of 1987 until May of 1988, approximately 142,000 gallons of oily wastewater was processed and discharged through a second oil separator without compositing samples for analysis. This practice has since been discontinued. On October 4,           1988, the oil and grease maximum limit was exceeded due to a malfunctioning polymer addition pump. The pump was repaired and th6 system returned to compliant operations.           On December 6, 1988, a bypass of outfall 201 occurred when an unknown                   )'
amount of condenser cooling water was released y                                                       to the storm drain system when a valve f ailed to seat completely during essential maintenance of Unit I condenser tubes . A plant modification has since been initiated which will eliminate future releases of this nature.

Due to the system operational problems discussed above, a system operation and design evaluation was conducted in 1988. Several corrective actions were implemented consequent to recommendations made in the evaluation.
These actions included modifications to the gross oil separator level control switches, installation of new Trice 11erator skimmer blades, repairs to the gross oil seperator belt skimmer, installations of a manual throttle valve between the separator and the Trice 11erator, replacement of aerator pump diaphragms, use of alternative treatment chemicals, and retention of a contract for emergency surge tank service for oily wastewater. Actions also included increased monitoring and' identification of sources of input to the treatment system and the manual removal of excess oil with the use of sorbent materials and portable skimmers.
D. Outfall 301 (East Sanitary Waste Treatment System)
Three (3) violations were incurred on outfall 301 in 1988. On April 15, 1988, a blockage occurred in the sanitary waste collection system which resulted in an overflow of approximately 50 gallons of sewage from a manway to the storm drainage system. The line was cleared and the system returned to compliant operations. On July 12, 1988, the TSS daily maximum was exceeded due to hydraulic overload of the plant attributable to higher than normal volumes of l    influent. Manual adjustments were made to I    reduce flow to the plant in order to return the plant to compliant operations. Due to the high TSS value in July, the daily TSS average for that month was also exceeded.
E. Outfall 401 (West Sanitary Waste Treatment System)              ;
Two (2) violations were incurred on outfall 401 in 1988. On October 4, 1988, the TSS daily                   l maximum was exceeded when an air lift pump became clogged resulting in carryover into the effluent tank. The obstruction was removed and                  {
the system was returned to compliant operations.                i On December 6, 1988, a frequency violation was                  !
incurred on this outfall when the offsite contract laboratory personnel did not analyze the BOD 5 sample within the required holding                    ,
time. No further violations occurred on this l

In Atigust,1987, the subject Administrative Order was issued as a result of alleged permit exceedences involving five outfalls at the South Texas Project. These outfalls include four sewage treatment systems (Outfalls 002,301,401,601) and a,n oily waste treatment system (Outfall 201). In response,                                                      submitted on September    29, 1987,    Houston Lighting & Power Company (HL&Padetaileddescriptionof to resolve problems that resulted in the permit exceedences cited in the Order. Since that time,. HL&P has taken additional steps to ensure permit compliance is maintained at the South Texas Project. These measures include creation.of a task force to identify necessary corrective actions, treatment system design and operation review by technical consultants, and implementa-tion of various treatment system equipment and operational improvements.
These actions are described in detail in the attached Compliance Status Report dated February, 1989. In addition, the report discusses the compli-ance status of all other STP outfalls and addresses wastewater bypasses and overflows that occurred in 1987 and 1988. Measures taken by HL&P to prevent additional bypasses or overflows are also described. An earlier version of this report was reviewed with EPA staff in December,1988.
As noted in the Compliance Status Report, HL&P was successful in reducing sewage treatment system parameter exceedences by 75 percent from 1987 to 1988 and in reducing oily waste tg atment system parameter exceedences by 63 percent over the same period. The corrective actions described in the Status Report were completed by October, 1988. During the following five-month period (October,1988 - February,1989), Outfalls 101, l     301, Sol, and 002, have operated with no permit exceedences or by] asses.                                    '
Outfalls 001 and 501 were inactive during this )eriod. As descriaed In the Compliance Status Report, Outfalls 201 and 401 lave each incurred minor isolated upset conditions or equipment malfunctions during this period, but have otherwise demonstrated compliant operation. All outfalls have operated with no permit exceedences or bypasses in 1989.

on the day of the exceedence the chlorine residual was 1.0 mg/1, which is in compliance with TWC permit requirements. Settleable solids levels were higher than normal and plant personnel believe this caused sludge carryover and resulted in elevated fecal coliform levels. The resulting free residual chlorine available at this time was not adequate to effectively treat the fecal coliform present. Settleable solids levels have subsequently been reduced by wasting to the activated sludge holding tank and the system returned to compliant operation.            No adverse impacts are anticipated as a result of this exceedence.

Houston Lighting & Power company is providing the following supplemental information concerning the above-cited exceedence which was initially reported on February 2, 1988 (see attached notification).                                   Upon further investigation, it has f bun datermined that equipment malfunction was not the primary cause but rather
! inidsquate response time on part of the operator in isolating the tank when notified that filling was complete. Additional administrative controls are being instituted to ensure that plant communications relating to such operations are made end rssponded to in a more timely and occurate manner. As previously reported, no cdvarse impact to the receiving stream is anticipated as a result of this release.

Approximately 30 gallons of regenerate wastewater overflowed from an equipment pad drain to the storm drainage system. The overflow occurred when an automatic valve did                                                       !
not fully open when transferring wastewater to the neutralization basin, causing the water to back up and overflow the equipnent drain. The valve was manually opened upon discovery and the overflow halted. The pH of the water in the receiving drainage ditch was 7.8 standard units.            No adverse impact to the environment occurred.

As a result of a valve inadvertently left in a partially open position, approximately two thousand (2,000) gallons of condensate grade water was allowed to                                                                 (

The operator noticed visible suspended solids and placed the system into recirculation immediately halting the discharges. Discharge samples ware collected a short time later. Duplicate compliance samples yielded results of 30 mg/l and 11 mg/1. An investigation of analytical quality control is underway to varify validity of analytical results.
In addition, a licensed waste oil recovery contractor was utilized to drain the cyatsm of all oil and sludges.                Efforts are also underway to identify any sources of cxcoss oil to the system.

Malfunctioning level switches on the Gross Oil Separator are thought to have caused the exceedence of the oil and grease parameter.
On the date of the exceedence, visual observation indicated that the system was operating properly. Modifications to the level switches have been approved. In the interim, compensatory operatur action is being performed. Subsequent sample analysis (6/19/88) indicated that the system was in compliance. No adverse impacts to the environment were observed.

Outfall 201 floor drainage discharges normally occur through the main plant' floor drainage treatment system. This system has been experiencing periodic hydraulic treatment upsets in conjunction with generating unit start-up activities. During these intermittent upsets excess floor drainage was trucked from the main plant system to a second oil separator. An estimated 142,000 gallons of floor drainage was treated and discharged through this second source during the period from November, 1987 until May, 1988.
The discharge permit provides that multiple source discharges should be composited for analysic and reporting. Although compositing was not performed, the discharge was believed to be compliant. No trace oil or evidence of potential adverse impacts were observed in the plant drainage system and receiving water (Segment 1401).                                                                                             I Corrective actions have been taken to prevent recurrence of the above condition. A TWC waste control number has been obtained to allow offsite shipments of oil contaminated wastewater. Near-term treatment system upsets will be resolved by offsite disposal using licansed contractors.
Additionally, Pollutica Control Engineering Inc. has been retained to evaluate treatment syn um perfo;mancy. Future compliance will be assured by the 1:nplementation of trestment system performance enhancemer,ts, and by the operational fluibility avr.11able tMouch offsf.te cisposal .

A l_             clogged anionic polymer pump is believed to have caused carryover of emulsified oil and resulted in an elevated oil and grease level. The polymer pump was unclogged and the system returned to compliant operation. No adverse impacts to the environment are anticipated as the result of this exceedence.

This condition was caused by failure of a circulating water pipe valve to seat completely and isolate cooling water flow' The floor drainage collection system was unable to contain the leakage.
An equipment modification which will alleviate future releases of this nature is under development and will be completed later this year. In the interim, excess floor drainage will be monitored in accordance with the permit parameters for Outfall 201.

An estimated 50 gallons of sewage overflowed containment and escaped off-site. The ovarflow occurred as the result of a blockage which developed in the collection system.
The wastewater overflowed to the storm water drainage system. The overflow was halted soon after it was discovered and efforts were made to disinfect the portion of the storm                                                     !
water drainage system where the overflow occurred. No adverse impacts to the receiving stream were observed as a result of this release.

On the day of the exceedence, an air lift pump in the clarifier of the west sanitary waste treatment system was found clogged. This contributed to sludge carryover to the effluent tank and resulted in an elevated TSS level.                              The air lift pump was unclogged and the system returned to compliant operation. No adverse impacts to the environment are anticipated as the result of the exceedence.

PART II OTHER CONDITIONS
: 1. There shall be no discharge of polychlorinated biphenyl transformer            j fluid.
: 2. The " daily average" concentration means the arithmetic average (weighted by flow value) of all the daily determinations of 1
concentration made during a calendar month. Daily determinations of concentration made using a composite sample shall be the concentration of the. composite sample. When. grab samples are used, the daily determination of concentration shall be the arithmetic             !
average (weighted by flow value) of all the samples collected during that' calendar day.
The " daily maximum" concentration means the daily determination of concentration for any calendar day.
: 3. For the purposes of this permit, daily temperature discharge is defined as the flow weighted average temperature (FWAT) and, on a daily basis, < shall be monitored and recorded in accordance with          3 Part III, Section C, of this permit. FWAT shall be calculated at equal time _ intervals not' greater than two hours. The method of

: 4. The term " total residual chlorine" (or total residual oxidants for intake water with bromides) means the value obtained using the amperometric method for total residual chlorine described in the latest edition of " Standard methods for the Examination of Water and Wastewater".

: 5. The term " metal cleaning wastes" shall mean any cleaning compounds, rinse waters, or other waterbone residues derived from cleaning any metal process equipment including, but not limited to, boiler tube cleaning, boiler fireside cleaning and air preheater cleaning.
: 6. The term " low-volume wastesources" means wastewaters from, but not limited to: wet scrubber air pollution control system, ion exchange water treatment system, water treatment, evaporator blowdown, laboratory and sampling streams, floor drainage, cooling tower basin cleaning wastes and blowdown from recirculating house service water systems.
: 7. In accordance with information submitted by HL&P on June 28, 1982, i                          the intake structure is approved as Best Available Technology in accordance with Section 316(b) of the CWA.
: 8. Outfall 001 blowdown is limited to 12.5 percent of the difference between the Bay City gage flow and makeup diversion for the STP.
During June, July, and August, the percentage is limited to 10 l
percent. . Blowdown is permitted only when the remaining river flow, l                          after makeup diversion, is 800 cfs. After the allowable blowdown is determined, the reservoir water level and the river flow opposite the point of blowdown will serve as constraints to determine whether                                      ;
the blowdown is allowed.        If either the reservoir water level is less than elevation 38 MSL or the tidal river flow at the point of blowdown is less than a prescribed value, blowdown is not permitted.
Blowdown is limited to a maximum flow of 308 cfs.
: 9.   "The required number of ports through which discharge occurs is a function of blowdown rate as shown in the chart below. These blowdown conditions are described graphically on pages 3 and 4 of Part 11.                                                                          ,,

: 12. CHRONIC BIOMONITORING REQUIREMENTS - OUTFALL*001
: a. The permittee shall test the effluent for toxicity in accordance with the provisions in this section. Such testing will determine if an appropriately dilute effluent sample affects the survival and reproduction or growth of the appropriate test organism. The permittee shall initiate the following series of tests within 60 days of the effective date of this permit to evaluate wastewater toxicity. All test organisms, procedures, and water quality assurance criterion used shall be in accordance with the latest revision of "Short-Term Methods for Estimating the Chronic Toxicity of Effluents and Receiving Waters to Marine & Estuarine Organisms," EPA-600/4-87/028 which is currently in draft form. The following tests shall be used:
: 1) Chronic static renewal 7-day survival, growth and fecundity test using Mysidopsis bahia (Method 1007.0).
: 2) Chronic static renewal 7-day larval survival and growth test using sheepshead minnow Cyprinodon variegatus (Method 1004.0).
: b. A minimum of 5 dilutions must be performed in addition to an appropriate    j i
control, using a minimum dilution f actor of 0.5, i.e.100%, 50%, 25%,12.5%     ,i

and 6.25%.
: c. The samples shall be collected at a point following the last treatment unit. Dilution water used in toxicity tests will be receiving water outside the area of discharge. If receiving water is unsatisfactory as a result of pre-existing toxicity (greater than 20% mortality in the control),
the permittee must substitute reconstituted dilution water, inth salinity similar to that of the receiving water. The permittee shall also report to EPA the toxicity of the receiving water.       .;
: d. A minimum of three 24-hour composite samples must be collected so that the maximum holding time for effluent samples shall not exceed 72 hours.
Samples shall be chilled to 4 degrees Centigrade when collected, shipped, and/or stored. The toxicity text must be initiated within 36 hours after initial sample collection. The test solutions shall be renewed daily. The term "24-hour composite sample" means a sample consisting of a minimum of 3 grab samples of representative effluent collected at regular intervals over a normal 24-hour operating day and combined in proportion to flow or a sample continuously collected in proportion to flow over a nonnal 24-hour operating day. Representative grab samples collected ouiing periods of chlorination may be substituted for ficw weighted composite samples.

: e. The toxicity tests specified in paragraphs (a) and (b) above shall be conducted once per quarter.      The permittee shall prepare a full report of the results according to EPA 600/4-87/028, Section 10, Report Preparation.
I                         This full report need not be submitted unless requested and shall be retained following the provisions of Part III.C.3. of this permit.
: f. The permittee shall submit the toxicity testing information contained in Table 1 of this permit to EPA along with the Discharge Monitoring Report (DMR) submitted for the end of the reporting period following the toxicity test.
: g. Should no toxicity occur within the first year of toxicity testing, in accordance with paragraph (h) below, for both species tested at the I                         effluent dilution equivalent to double the concentration of effluent after critical dilution, the permittee shall certify this information in writing to EPA Region VI and these biomonitoring requirements shall expi re .
: h. For the purpose of this biomonitoring requirement, chronic toxicity is defined as a statistically significant difference at the 95% confidence level between the survival and growth or reproduction in the appropriate                     ;,
test organism exposed to the control and to an effluent dilution.
                                ,   This permit shall be reopened to require further monitoring studies ar1/or effluent limits if biomonitoring data show actual or potential anient toxicity to be the result of the permittee's discharge to the rt:eiving water. Modification or revocation of the permit is subject to t!e provisions of 40 CFR Part ]?2.62. Accelerated or intensified toxicity tt sting may be required in accordance with Section 308 of the Clean Water Act .
