R:pstt No. PEl-TR-870200-02 Page No. 1 I                                                                            INTRODUCTION 1.0                              objective The ob jec tive of     the   test      program was     tv demonstrate    the operability of Limitorque control and power wiring and Okonite tape splices , when subjected to a simulated steam accident and functional sisulation. The test was performed per Procedure ptI-11t-870200-01 presented in Appendix 1.
2.0                             Equipment Description The test as sembly simulated control and                power wiring inside Limitorque_ valve ac t uat or s .      The   test assembly consisted of two circuit loops inside a NEMA-4 enclosure. The test specimens were assembled by Cooper Nuclear Station (CWS) personnel and were then 9                                                     sent to Patel Engineers where the specimens were installed into the NEMA-4 enclosure.      The enclosu re simulated the timitorque'

housings    at CNS. The  test   assemblies were f abricated per Figure g                                                     1 of Appendix I.
o-                                                    The two circuit loops were labeled Loop 1 and Loop 2. Loop 1 contained two segments of Limitorque switch (control) wire (black
(                                                  braid), naturally aged during 12 years of service at CNS. Loop 2 contained two segments of Limitorque motor (power) lead wire (yellow braid), naturally aged during 12 years of service at CNS.
s                                                     Circuit Loops 1 and 2 each contained - three bolted Okonite T35/T95 tape splices. The lead wires used for Loops 1 and 2 wore
-                                                      Rockbestos Firewall SIS 16 AWG 600V Nuclear Cables. Per CNS, the Okonite splices were assembled by CNS personnel in accordance N                                                     with CNS Procedure 7.3.26.

Equipment identification shall be verified and the equipment tagged for future verifications.

111.3.1  Visual Inspection This visual inspection        is documented in Appendix III.             Upon removing the NEMA-4 enclosu re from the test chamber, no visual damage was noticed in Circuit Loop 1.               Examination of Circuit Loop 2 revealed the cause of the low IR me asu rement s . Wire Specimen "C" (see Appendix 1. Figure 1) was frayed, exposing the conductor.      Discoloration of        the enclosures ' inner surf ace was visible in the area where the            frayed wiring was touching        the enclosu re wall. This indic ates that               arcing had taken   place during the testing. Wire Specimen "B" (see Appendix I, Figure 1) appeared - to be damaged siallarly to Specimen "C" but damage was not as extensive.
Further testing of Circuit Loop 2 revealed that when Specimen "C" was separated from the enclosure wall, the IR remained the same, indicating another leakage point. When Specimen "C" was pressed g                 agains t the enclosu re well, the IR went to 0, dead short. And when Specimens "B" and "C" were both separated from the enclosure-                   *
    -                   wells, the IR increased to 40 megohne (Appendix III).
O                 A surf ace layer of water was noticed in the bottom of the NEMA-4 enc losu re . Through evaluation of data and other observations , it' O                 is believed the NEMA-4 enclosure was completely submerged during some portion in the test.                This is evidenced by the drop in 4-               temperature, below the required 230'F at 91 hou rs , 58 minutes into the test (see Appendix II, page 25 of 25).

l The leakage current data were recorded using a datalogger and a four channel pen type recorder was used to record the chamber temperature and pressure.                                                                                                                                                  i s.,            The instrumentation used to measure the above parameters was calibrated using secondary standards that are traceable to N8S. The equipment and
     -             instrumentation calibration date, accuracies, manufacturer and identification number are shown in Appendf x. 3 of this report.                                                                                                                                                                                   ,
     .o            Baseline Functional Test The test specimens were subjected to a functional test in accordance with.                                                                                                                                                         f Iho-               Patel Engineers Test Procedure Number PEl-TR-870200-01, paragraph 3.1, prior j
to the accident test.                                                                                                                                                                                                              i Accident Simulation Test Loop 1 was energized with a 120 VAC, 60 Hz, single phase potential and loop '2 was; energized with a 480 VAC 60 Hz single phase potential throughout the                                                                                                                                                           i accident test. The current leakage for ea% loop was measured from conductor                                                                                                                                                       I to ground as shown in Figure 2, Appendix 1 of this report.
During the accident test the leakage current was recorded every 10 seconds for                                                                                                                                                    l the first five minutes of the test, every minute for the next 30 minutes, and every 20 minutes thereaf ter.
l            The chamber temperature and pressure were continuously recorded using twoAt                                                                                                            (2)55 thermocouples and one pressure transducer throughout the accident test.
