Dear Sir or Madam:
In accordance with 10 CFR 70.74(b), Global Nuclear Fuel-Americas, LLC (GNF-A) hereby submits a written follow-up report for Event Notification 57474 that was provided to NRC on December 17, 2024 (Reference 2).
Event 57474 As discussed in the initial event report, GNF-A reported an injury resulting in partial amputation to the North Carolina Department of Labor pursuant to 29 CFR 1904.39. Consistent with 10 CFR 70, Appendix A(c), a concurrent report was provided to NRC and is included as to this letter.
Additional information is provided as follows:
Event Details and Safety Significance At approximately 1655 EST on December 16th, 2024, the North Carolina Department of Labor was notified that an employee was injured while operating an electric powered pallet jack resulting in partial amputation of left thumb soft tissue.
Immediate Corrective Actions Taken The employee was inside the airborne controlled area when the injury occurred. The employee was surveyed, free released with no contamination and transported offsite for medical treatment.

US NRC M250042 February 7, 2025 Page 2 Probable Cause of Event The employee was inexperienced in operating the powered pallet jack and was not observant of the location of a nearby I-beam. The steering and drive control handlebar struck the I-beam crushing the left thumb between the handlebar and I-beam.
Short Term Corrective Actions The pallet jack was verified to be operating correctly. A safety standdown was conducted with Fuel Manufacturing Operations (FMO) operation and maintenance teams. The learning opportunity was leveraged and shared with other work areas.
Longer Term Corrective Action North Carolina Department of Labor closed its review of the injury on December 27, 2024.

Because the North Carolina Department of Labor was notified, a concurrent notification to the NRC Operations Center is being made per 10 CFR 70, Appendix A(c).
