No At approximately 0720 on 2/18/2020 the K-1f environmental air sample pump was found in a degraded condition during the weekly filter change out.

The pump sounded as if it was failing and the flow indicator was not indicating any flow.

Turned pump off. Restarted the pump and did not see any improvement. Turned pump off again.

Changed out filter.

Contacted system expert and informed of issue.

Noted issue on sample collection data sheet.

Repair or replace pump. This is currently in progress.

Create a CA to include this CR in the 2020 Annual Environmental Report.

N/A No Environmental air sampler (#6737) at location K-8 was found off with blown fuse. After the fuse was replaced, the pump was turned on and the motor would start but was seized up. The pump was immediately turned off prior to blowing a second fuse. The air sampler had operated for 44.34 hours3.935185e-4 days <br />0.00944 hours <br />5.621693e-5 weeks <br />1.2937e-5 months <br /> prior to the fuse blowing. The air sample filter was changed out and will be sent to the vendor for analysis with a reduced sample volume.

Removed K-8 environmental air sampler from service and replaced it with a calibrated air sampler.

Create a CA to include this CR in the 2020 Annual Environmental Report.

N/A No While performing the weekly Environmental Air Sampler particulate filter change on 4/28/20, the air sampler head at location K-2 was found on the ground in front of the environmental air sampler. The air particulate filter was not discolored, indicating that the sampler head most likely fell off shortly after being changed on 4/21/20, and an environmental air particulate sample was not collected at location K-2 (control) for the week ending 4/28/20.

The most likely cause of the air sample head falling off the air sampler was due to the air sample head not being firmly latched in place after being exchanged on 4/21/20.

Installed an air sample head with a new particulate filter on the environmental air sampler at location K-2 in accordance with RP-KW-HSP-HPE-1164, Environmental Sample Collection. Verified that the air sampler head was latched in place.

Discussed event with RP personnel and reinforced the expectation to verify that the environmental air sampler head is latched firmly in place after being exchanged, as part of the post-filter change operational check required by RP-KW-HSP-HPE-164, Environmental Sample Collection.

Initiated CR to document the event.

No On July 14th at 0745 the K-1f environmental air sampler was found off. Upon investigation of the issue the power to the Maintenance Material Storage Building was found off, which supplies power to the air sampler. The approximate time of this happening based on the reading from the Run Time Indicator was 0000 Monday.

Informed the ISFSI Shift Supervisor (ISS).

Changed out the sample filter.

Noted the problem on the Environmental Sample Collection Data Sheet.

Create corrective action to include this CR in the 2020 Annual Environmental Report.

N/A No During the collection of the weekly environmental air samples, the run time totalizer indications at locations K-43 and K-8 were blank (no indication of run time displayed). The air sampler flow rate indication was satisfactory (30 LPM) and the sample filter loading was normal at K-43 and K-8 (i.e., filter discoloration was similar to the sample filters collected at location K-1 f and K-2). Pre and post filter change out operational checks were satisfactory.

Calculated total run time for the weekly air samples at K-43 and K-8 using filter service (change) dates/times and documented the results on RP-KW-HSP-HPE-164, Attachment B, Environmental Sample Collection Data Sheet.

Initiated CR to document event.

Notified RP personnel responsible for instrument calibration and maintenance.

Replace the environmental air sampler run time totalizers at locations K-43 and K-8 following weekly collection/change out of the air sample filter during the week of August 10, 2020, and calculate the total run time for those weekly air samples using the filter service (change) dates/times.
