ENCLOSURE 1 NOTICE OF VIOLATION Westinghouse Electric Corporation Docket No. 70-1151 Columbia Nuclear Fuel Plant License No. SNM-1107 During the Nuclear Regulatory Commission (NRC) inspection conducted on July 28, 1986 - August 1,1986, a violation of NRC requirements was identified. The violation involved failure to maintain and follow approved radiation protection procedures. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is listed below:

License No. SNM-1107, Condition 9 required that licensed materials be used in accordance with statements, representations, and conditions contained in

., Chapters 2, 3, and 4 of the application dated March 26, 1984, and supplements thereto.

Paragraph 3.2.1.1 states that written procedures describing general radiation protection requirements shall be maintained and followed.

Contrary to the above, the requirement that procedures be maintained and followed was not met in that:

1. Between March 21, 1986, and July 14, 1986, a review of survey records identi#ied 18 occurrences of contamination in excess of the 1000 o'ln/100 cm2 action level on the controlled side of the main step-off pad at the Controlled Area entrance, and decontamination was not performed within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> or three working shifts as required by Health Physics Operating Procedure 05-014, Revision 5, Section 7.5.1.
2. Between July 28 and August 1, 1986, individuals exiting the Controlled Area at the main step-off pad, the change rooms and the centractor outprocessing area were observed by inspectors not to follow the posted instructions for monitoring (frisking) as required by Regulatory l Affairs Procedure RA-200, Revision 5, Section 2.3 and RA-203, Revision 6, Section 2.12. Individuals held the instrument probe at distances greater than the specified 1/8 to 1/4 inch from their bodies and moved the probe in rapid, sweeping motions, such that the instrument would not detect the contamination limit.
3. Regulatory Affairs Procedures RA-200 and 203 did not specify the requirements for monitoring by personnel exiting " limited areas.f such as the Chemistry Laboratory thus are inadequate to assure monitoring of personnel leaving the area.

4. Health Physics Operating Procedures01-008, PIC-6A, 01-09, Geiger Counter E-120, and 01-10, Geiger Counter Model E-140, only required that the instruments " read acceptably close to the source" and did not specify an acceptance criteria or tolerance on the response to the source and thus were inadequate to assure proper response of the instruments to a radioactive source prior to use.

This is a Severity Level IV violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Westinghouse Nuclear Fuel Division is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:

(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
