In accordance with 10CFR55.41, I am notifying you of a condition that could affect the perfonmnce of my duties as licensed by Operators License number OP - 20321, Docket nunber 55-20436.
I have been diagnosed as recmmended for corrective lenses for distant vision. Although my vision at 20-30 feet became slightly blurred, my normal range of vision in the control room (6-12 feet) was unblanished.
