INTEROFFICE TASK GROUP ON PERFORMANCE INDICATORS - MEETING MINUTES AND NOTICE During the meeting of July 8 and 9,1986, we updated the performance indicators (PIs), lead responsibilities for confirmation / validation, data period, and data sources for the trial program as indicated below.

- 17. Overall Maintenance NRR/DHFT NRR/DHFT Indicator The discussions and commitments from the meeting are summarized below.
PI #1:
Scrams - Upon review of the data, it was decided to use scrams from
>15% power (PI #1A) and scrams from <15% power (PI #1B) for the trial program.
PI #1A would be trended quarterly with 4 quarter moving average and on per 1,000 critical hours basis.
PI #1B would be the number of scrams from <15% power with rod motion per quarter and trending would include 4 quarter moving average. AE00 would try out average value plus one time (instead of 2 times) the standard deviation for alert levels and report in the next meeting.
PI #2:
ESF Actuations - This PI would be trended quarterly with 4 quarter moving average. AE0D is to further refine the 1984 data for develop-ing alert levels etc. and analyze 1985 data for the next meeting.

PI #3:
Safety System Actuations - Same as PI #2.
i PI #4:
Significant Event Frequency - This replaces former PI #4, Transients.
RES reported that the EG&G report discusses transients leading to scrams only and, as such, PI #1 should be sufficient.
The new PI #4 includes degradation of important safety equipment, unexpected plant response to a transient, discovery of a major condi-tion not covered in safety analysis, and degradation of fuel integ-rity, primary coolant pressure boundary, or primary containment boundary.
Preliminary evaluations indicate that PI #4 is not covered well in other PIs.
NRR/0 RAS is to further develop this indicator and report in the next meeting.
PI #5:
Safety System Failures - The data available contain actual or potential loss of a system safety function.
NRR/0 RAS is to further analyze the data and report in the next meeting.
PI #6:
Unplanned Shutdowns - There were no data available for discussions.
RI is to analyze the data with assistance from PNL and report in the next meeting.
PI #7:
Causes of Events (Overlay on PIs #1, 2, 3, 5 and 6) - Preliminary evaluations indicate that additional work is necessary to recategorize the causes.
IE/D2FER to continue this work and report in the next meeting.
This PI will be trended quarterly with 4 quarter PI #8:
Forced Outage Rt moving average similar to scrams.
AE00 is to further refine the data and report in the next meeting.
PI #9:
LC0 Action Statements (LC0 Hours for Selected Systems of Selected Plants) - This PI will be trended on a monthly basis for the quarter and the reasons will be categorized as planned or unplanned.
- Also, the number of LCOs on a monthly basis will be trended.
Regions will provide additional data for the next meeting. AE00 is to look into NPROS for the same plants and period for correlation and report in the next meeting.
PI #10:
Fraction of Control Room Alarms That are Continuously Alarming (Off-normal) Above 70% Power - The data collection by resident inspectors for a plant generally took an hour and data were already being col-lected for many plants.
Regions will provide additional data for the next meeting.
PI #11:
Average Age of Outstanding Audit Items - It was noted that it took some licensee 2 man-days to collect the data for a plant.
It was also reported that at many sites it is difficult to separate the QA audit items from others.
The regional task group members are to provide additional guidance in this area.
It was agreed to trend this PI monthly with some moving average.
Additional data are to be provided for the next meeting.

PI #12:
Maintenance Backlog - The only data presented were from Region III.
The data did not include preventive maintenance.
Regions are to provide additional data for the next meeting.
PI #13:
Enforcement Action Index - Regions presented some data for discussion.
PNL is to continue working on this PI.
The PNL 766 file needs to be compared against the Headquarters file for the last quarter of 1985 for accuracy.
In addition, Severity Level 1, 2, and 3 violations need to be compared against Enforcement History files.
Additional data will be provided for the next meeting.
PI #14:
Total Integrated Exposure - It was generally agreed that trending of this PI on a quarterly basis is not meaningful and that the data should go into SALP.
In addition, this is more of a health physics indicator than plant performance.
However, the trending of the data will be available for the next meeting.
PI #15:
Turnover Rate / Vacancies - Some data were presented for discussion and additional data will be made available for the next meeting.
PI #16:
Average Years of Experience for R0s/SR0s, Number of Fully-Staffed Operating Shifts, Number of Active Licenses 'It was noted that it was difficult to obtain the data at some sites.
Regional task group members are to provide guidance as necessary.
NRR/DHFT is to look into GL 84-16(?) for the availability of such data and report in the next meeting.
PI #17:
Overall Maintenance Indicator - The trending will be done quarterly for each plant.
NRR/DHFT and PNL are to continue to work on the data and report in the next meeting.
Other assignments and items of interest:
It was generally agreed that the performance model and PI logic model provided by RES were the right way to look at the PIs.
RES (Johnson) is to continue to refine the logic model including the display of the relationship of PIs to risk.
AE00 (Dennig) presented a report on the feasibility of using NPRDS data for Safety System Unavailability.
It was agreed that NPRDS can and should be used for PIs as long as we recognize the risks involved.
Safety System Unavailability should continue to be considered as a future indicator.
AE00 (Dennig) is to provide a write-up on using the 472 component failures analyzed by INEL. The status of the work is to be reported in the next meeting.
Maintenance Performance Indicator should remain under consideration for future Pl.
RES (Johnson) is to continue working on it.
There was no update on using Drawing Status as a PI.

Several comments were received on the SECY outline.
IE/DEPER will continue to work on it and present write-up, as appropriate, for the next meeting.
The next two task group meetings have been scheduled for August 5 and 6,1986, and August 19 and 20, 1986. Both meetings will start at 8:15 a.m. and will be held in MNBB-6110. The August 5-6 meeting will focus on the evaluation of final analyzed data from AEOD and raw data from others, and on the SECY write-up. All data should be available for the meetings.
The August 19-20 meeting will focus on the evaluation of the remainder of the final analyzed data and on the SECY write-up.
