TASK ORDER NO. 9 ENTITLED, "lNSPECTION. REPORT CODING FROM JUNE 6,1999 TO APRIL 30,2000," UNDER CONTRACT NC. NRC-03-97-022

Dear Mr. Nock:

In accordance with Section G.4(c) of the subject contract, entitled " Task Order Award," this letter definitizes the subject task order. This effort shall be performed in accordance with the enclosed Statement of Work. Task Order No. 9 shall be in effect from April 28,1999 through April 30,2000 with a cost ceiling of $42,885.61. The amount of $40,080.01 represents the total estimated reimbursable costs, the amount of $2,805.60 represents the fixed fee. The obligated amount of this task order is $39,934. This amount shall not be exceeded until notice is provided to you that additional funds are available. It is estimated that this obligated amount will cover performance of work through March 2000. Lirispection Report Coding from June 6,1999 to April 30,2000 ' Technical Monitor: ; Mary Ann Biamonte, NRR (301) 415-1073 ~ TAC Number:1 ' M 79721; Priority Number: V c-" 2-Backoround Over the past four years, the NRC Human Factors Assessment Branch has implementeo the Human Factor Information System, in order to keep the HFIS data up to date, the Inspection reports from June 6,1999 to April 30,2000 must be reviewed and coded. ) Objective - ) The objective of this task order is to obtain assistance from ORTECH for the review and coding ofinspection rep s beginning June 6,1999 to April 30,2000. Inspection re@ts need to be reviewed using t e HF!S Coding instructions. Technical and other Soecial Qualifications Reouired One nuclear specialist with expertise illhuman performance for review and coding of human performance issues in inspection reports. t It is the responsibility of the contractor to assign technical staff, employees, or specialists who have the required educational background, experience, or combination thereof to meet both the l technical and regulatory objective of the work specified in this statement of work. The NRC will l rely on representations made by the contractor concerning the qualification of the personnel assigned to this task order including assurance that all information contained in the technical and cost proposals, including resumes, is accurate and truthful.

,W %,, 1} ~ : Work Reouirements and Schedule The contractor shall provide the qualified pdrsonnel to complete the following tasks: Tasks Completion Schedule 1. Read and code information from Inspection reports collected each month inspection reports between June 6, must be reviewed and coded within the next 1999 and April 30,200 month. Level of Effort The level of effort is estimated at 15 data entry staff weeks. Period of Performance The period of performance is from June 6,1999 to April 30,2000. Meetinas and Travel One trip per month from local contractor office to NRC Headquarters NRC Furnished Materials The contractor will be provided either a paper copy or an electronic copy of the inspection reports to be coded. l Q_eliverables l Human performance related information from inspection Reports coded on a database data entry sheet and delivered to the NRC Headquarters on the schedule defined in the schedule above. Other Aeolicable information The work specified in this statement of work is not licensee fee recoverable. 2 w}}
