o-SPECIAL REPORT LIFIED VAIVE 2NC32 (NC PORV)

Circumstances Initiating The Transient:

On June 16, 1986 during unit startup, valves 2NC-32 and 2NC-34 (pressurizer PORV's) were in the low pressure (L/P) mode. This reduces the lift setpoint of the PORV's to 400 psig. At 0053 hours6.134259e-4 days <br />0.0147 hours <br />8.763227e-5 weeks <br />2.01665e-5 months <br /> on June 16, 1986 "D" Reactor Coolant pump was started. Pressure increased from 330 psig to approximately 350 psig and stabilized at that point. At 0056 hours6.481481e-4 days <br />0.0156 hours <br />9.259259e-5 weeks <br />2.1308e-5 months <br /> "B" Reactor Coolant pump was started.

Two minutes later, valve 2NC-32 lifted for approximately 1 second.

Effect of PORV On The Transient:

After the PORV lifted for approximately 1 second, pressure decreased to approximately 350 psig. The remaining two Reactor Coolant pumps were started with no further difficulty.

Corrective Action To Prevent Recurrence:

A statement will be added to the limits and precautions section of the procedure for starting Reactor Coolant Pumps. This statement will caution the operators on pressure spikes that can result from starting a pump.
