Assessment Team Observations - Construction he Assessment Team observtc jacking operations for the CT 3 pier and the E5 grillage and a check of* the tension for the FIVP support tie rods. The rate, increments, and duration of loadirt conformed with the appropriate i procedures. When the' maximum allowable upward movement of the EPA occurred  !

during initial loading of the E5 grillage, Jacking loads were reduced to the specified value for this condition. Once Resident Engineering had evaluated

3 J.O. No. 14358 Midland Plant Unita 1 & 2 Independent Assessment of Underpinning frames. The Assessment Team believes that initiation of this review was responsive on the part of the Contractor. However, a number of findings resulted during'the review; including improper identification of material and storage areas, improper storage of material, and the use of personnel not trained to the procedures for handling and storage of materials. A QAR was initiated by the Contractor to document corrective action implemented to address the deficiencies noted in the review. It is the opinion of the Assessment Team that more diligence is required on the part of the Subcontractor and MPQAD to ensure that material storage and handling requirements are properly implemented to prevent the inadvertent use of construction materials which do not meet quality standards and to minimize material damage during onsite storage.

FSO has completed the June review of their controlled documents. All documents with attachments were reviewed. Out of a total population of 2,150 documents, 10 deficiencies were found. Eight of the deficiencies were due to missing attachments or inadvertent posting of unnecessary attachments to documents. The remaining two deficiencies were register related errors. The error rate of approximately 1'4 indicates that FSO is adequately maintaining their controlled documents.

While reviewing documentation to the WS grillagein preparation for loading, MPQAD determined that weld inspection of a base plate shear lug for one of the grillage

. support columns had not been performed. Inspection of the lug was initiated by MPQAD, but upon identification of an apparent preheat nonconformance, the inspection was not completed. Upon disposition of the nonconformance and liftin6 of the Hold Tag, FSO mistakenly thought that the inspection had been entirely completed as required.by MPQAD procedures and released the column for erection. Since the nonconforming material had been installed, a difficult and time consuming disposition was required. Had the MPQAD inspector completed his original inspection, this situation would not have occurred.

MPQAD responded to an Assessment Team question regarding the initiation, preparation and closing of three QC Inspection ~ Reports. The Assessment Team is satisfied that the Inspection Reports were prepared in accordance with the appropriate procedures.

However, the Assessment Team's question would not have been necessary if annotation of the documents was performed to clarify exceptional situations. The Assessment Team believes that QC inspectors should be cognizant that future reviewers of quality records may not be fully aware of exceptional situations and short notes included within the reports will facilitate future use.

The E/W5 grillages were ready for load transfer pending resolution of an NCR written on the inspection of a column shear lug.

Pier CT 3 will be available fcr load transfer on 6/19. However, pier CT 10 will be delayed until 6/20/84 awaiting upper leveling plate grout cure.

The load transfer sequence for the above mentioned pier grillages is CT 10, CT 3 and E/WS grillages.

Pier Kc8 load transfer was initated on 6/17184.

J. Fisher reported that excavation in the SWPS west access shaft began yesterday.

Grouting of the lower level strut plates in the vicinity of the SWPS northeast corner is scheduled to be completed today in preparation far excavation of pier No. 1. (INFORMATION ITEM)

J. Fisher stated that upon completion of initial load transfer for the CT 3/10

' piers and the E/WS grillages, the following work will commence:

1. Drifting to piers CT 5/8.
2. Mass Excavation between E/W5 and E/W8 grillages to El. 600'*.
3. Excavation of piers E/W3.

Item 92 Welding of the Lower Bearing Plates.

B. Metros discussed welding of the lower bearing plates for the piers. It was noted that during welding of the Kc5 lower bearing plates, some of the weld material _had been deposited within a small gap between two of the plates. The

Mr. Metros stated that if welding of these plates had been performed after load transfer, this situation would not have occurred. (OPINION ITEM)

Response Items Item 92 SWPS Access Area Erosion.

J. Fisher discussed the soil erosion in the vicinity of the access shaft which resulted from heavy rainfall on 6/17/84. Erosion of the in-situ sand and back-packing had ccourred in several areas adjacent to the access shaft soldier pile wall, especially in the' vicinity of the SWPS east side. A protective berm and sumps had been installed along the north side of the SWPS as a result of previous erosion associated with surface runoff from heavy rainfall. However, the protect-ive berm was not installed along the east side of the SWPS. Additionally the sumps were not activated soon enough to prevent water flow over or through the protective berm. Remedial action presently underway includes repacking all eroded areas behind the soldier pile wall with clay and/or sand as cirected by the resident geotechnical engineer. To guard against future erosion, the protective berm will be extended around the east side of the SWPS and more sumps will be added. Also, the addition of automatic float devices to the existing sumps is being considered.

FSO rield engineers have been reinstructed as to the need for close monitoring of the sumps during inclement weather to ensure that pumps are activated when needed. (CLOSES ITEM 92-3)

Item 92 Material Storrde and Handling Procedure Findings.

J. Fisher provided the Assessment Team with a copy of the FSO Quality Engineering audit report associated with the Subcontractor's material storage and handling procedures. The report findings and necessary corrective action will be discussed with the Subcontractor today. (CLOSES ITEM 91-23)

J. Fisher reported that Engineering is presently evaluating the possibility of extending the CT 5/8 drifts once these piers are constructed to allow access to the CT 6/7 piers. The original construction sequence showed that access to the CT 6/7 piers would be gained from the CT 13/15 drift. (INFORMATION ITEM) l l Item 92 NRC Monthly Meeting.

Item 92 Material Handling and Storage Findings.

W. Kilker requested the status of corrective action implementation to address the FSO findings associated with the underpinning Subcontractor's handling and storage of material. J. Fisher responded that the Auxiliary Building Sub-contractor is in the process of correcting deficiencies noted in the review.

Additionally, a QAR has been initiated to document corrective action undertaken.

FSO will conduct a similar review of the SWPS Subcontractor's storage activities.

Item 92 MPQAD Monitoring of Material Handling and Storage.

W. Kilker asked what are MPQAD responsibilities associated with monitoring of the Subcontractor's material handling and storage activities, (i.e. audit frequency, date of last audit) J. McMaster will respond. (OPEN ITEM)

Item 92 Impact of Ongoing Work on Jacking Activities.

D. Zito discussed the impact of current work activities on the upcoming jacking associated with the CT 3/10 piers and E/W5 grillages. It was noted that during recent rejacking of piers CT 1/12, an NCR was written because^of concern that concrete removal activities at the time of rejacking may have affected instrumen-tation readings. Mr. Zito asked what provisions will be taken to ensure that concrete removal associated with the turbine building / control tower contingency jack installation do not affect instrumentation readings during jacking of the CT 3/10 piers and the E/W5 grillages. J. McMaster stated that the underpinning Subcontractor's jacking procedure requires that the Resident Geotechnical Engineer designate which work activities can proceed when jacking is in progress. If the l-RGE determines that a particular work activity will affect either the jacking or instrumentation readings, that activitiy is discontinued until the critical stages of Jacking has been completed. Item 92 SWPS and Auxiliary Building Work Activity Packages.

-D. Zito requested the status of work activity packages submittals for mass excavation between the E/W 5 and 8 grillages and construction of the SWPS pier No. 1. J. Kelleher reported that the work associated with the mass excavation between these grillages had just been separated out from a larger work package which covered mass excavation between the E/W2 and 8 grillages. The work package was finalized yesterday and will be submitted to the Assessment Team today. The work activity package fbr construction of pier No.1 at the SWPS will be made

Item 92 Reorganization of the W'ork Activity Package Review Group.

D. Zito requested details regarding' reorganization of the work activity package review group. J. Fisher responded that the review group manpower has been reduced and the group is now functioning on a part time basis. The number of remaining work packages does not necessitate the need to maintain the previous

Item 92 W5 Grillage support Columns.

E. Cviki discussed the NCR resolution regarding the missed QC welding inspection of a base plate shear lug for one of the W5 grillage support columns. Although the field welding engineer report documented their inspection, this is not acceptable fbr QC documentation. As a result, Engineering reanalyzed the support column without the shear lug. The reanalysis indicates that frictional resistance at the column base is capable of resisting the design horizontal loading without the shear lug.

Based on _their reanalysis, Engineering dispositioned the NCR 'to allow loading of the W5 grillage without any rework. (INFORMATION ITEM)

Item 92 SWPS Underpinning Activities.

J.. Fisher reported that grouting of the lower strut plates in the vicinity of the SWPS northeast corner is scheduled for today. (INFORMATION ITEM) l Item 92 BWST Ring Beam Addition in Valve Pit.

J. Kelleher discussed modifications which will be implemented during concrete placement for the BWST ring beam addition within each of the valve pits. The use of a pea gravel mix will not be used for the ring beam addition as previously stated. Instead a grout mix will be placed in the top 6" to 12" of the beams to ensure full penetration in the placement area. As previously reported additional holes will be drilled in the valve pit roof slab to provide increased access for the concrete vibrators. (Item 90-9 remains OPEN) (INFORMATION ITEM) l New Items Item 92 FCR Task Force Update.

P. Majeski discussed the implementation status of recommendations developed by an i FCR Task Force. The recommendation which have been implemented include prioritizing l FCRs and designating suitable process times, establishing an average number of FCRs to be processed daily and formation of a planning group to identify areas in advance where an FCR will be needed. Drifting to pier CT8 has begun.

Load transfer for the E5 grillage was initated.

Placement of concrete was completed for the Kc8 to Kc9-drift invert.

Item 92 SWPS Underpinning Activities.

J. Fisher reported that the two lower strut plates in the vicinity of the SWPS northeast corner were grouted in preparation for excavation of the pier No. 1 drop pit. (INFORMATION ITEM)

New Items Item 92 FSO Quality Engineering Bi-Monthly Controlled Document Review.

J. Fisher reported that the FSO Quality-Engineering group will perform their bi-monthly review of the FSO controlled document station this weekend. (INFOR-MATION ITEM)

Response Items Item 92 BWST Ring Beam Addition in Valve Pit.

The Assessment Team has reviewed the FCR specifying use of a grout mix fbr the top 10 in, of the BWST ring beam additions inside the valve pits. Additionally, installation of additional drill holes in the valve pit roof slabs to provide increased access fbr vibrators during concrete placement fbr the ring beams has been verified . The Assessment Team concludes that implementation of these

Item 92 MPQAD Inspection Reports of Concrete.

In response to the Assessment Team request for additional inforrration concerning the voiding of a Q.C. inspection report caused by cancellation of concrete placement for the CT 1/12 piers , MPQAD has reviewed with the Assessment Team the sequence of initiation, preparation and closure of three QC inspection reports associated with batching of concrete for these piers.

Based on a review of the circumstances and chronology of events associated with these inspection reports, the Assessment Team concludes that these reports were properly completed in accordance with MPQAD procedures. (CLOSES ITEM 91-21)
