i ATTACHMENT A FINDING:
10 CFR 50, Appendix B, Criterion V and the GPU Nuclear Corporation Operational Quality Assurance Plan, Section 6.11, require, in part, that activities affecting quality be accomplished in accordance with instruc-tions, procedures,or drawings.
Contrary to the above, as of July 8,1983, an activity affecting quality was not performed in accordance with procedures in that pipe support anchor bolts associated with Task RM-13H were not installed in accordance with GPU Specification SP-110lX-503, Revision 3, and Mechanical Corrective Procedure 1410-Y-61, Revision 0 resulting in selected anchor bolts being located less than 16 stud diameters from the edge of a concrete surface.
This is a Severity Level V violation (Supplement I).
RESPONSE
I.
Corrective Steps Taken and Results Achieved l
l Following the NRC inspection in which the violation was noted, Quality Control l
(QC) reinspected the pipe supports associated with Task RM-13H.
Nineteen (19) supports were found to have anchors which deviated from procedural spacing requirements.
Types of spacing deviations include anchor to concrete edge, anchor to embedded items, and anchor to anchor.
The non-conformances were documented on Material Non-Conformance Report Number 0201-83; engineering evaluation determined that sixteen of the supports are acceptable as-is with the remaining three supports requiring rework.
Although the installer was responsible for determination of exact support locations, most of the anchor spacing conditions were pre-determined by specified support spacing criteria, pipe elevations, and routing in close proximity to wall penetrations, embedded items and other equipment.
Devia-i tion from procedural anchor spacing requirements was unavoidable in many I
cases.
II.
Corrective Steps Which Will Be Taken to Avoid Further Violation Production personnel will be instructed to obtain engineering evaluation and direction prior to anchor installation in order to prevent recurrence of the violation.
