Dear Sir:
The NRC was notified of a potentially reportable deficiency in Reference A, Interim Reports were transmitted by References B and C.
At that time, it was estimated that a Final Report would be available by February 15, 1984 Due to the extensive investigation and evaluation required, a revised Interim Report is attached. It is now expected that this information will be finalized by May 21, 1984, at which time a complete report will be submitted..

. was discovered two '(2) Type N. insert plates were installed without the four (4).1"O x 9" Ig. anchor bolts (NCR CC-4201).

As a precautionary. measure,-approximately 10% of Units 1 and 2 wall embed plates were randomly selcted by Engineering at each level of each building for field inspection. However, additional embed plates at each building for'both units were also inspected during the 10% inspection process. :Furthermore, all wall embed plates with. critical loadings were also inspected in Unit 2 Auxiliary Building.- The resslts of the inspections are sutimarized as follows:

APPROACH TO AND STATUS OF' PROPOSED RESOLUTION
- An Engineering walkdown team was formed to investigate embedded
. plates listed above to determine ~ the structural adequacy of the cited embed plates. The results to date ccn be summarized as follows:
A.
Unit 3 IS. The anchor bolts with visible insufficient thread engagement were plug welded.in accordance with Section
~7.4~of Spe:ification No =13-CM-308.

Embed plates with-identified missing _ bolts (1 or 2 4
- missing: bolts per plate) were evaluated individually
^
- and found to.be struct'urally' acceptable as the "as built". plate' desiRn capacities are higher than the applied loads. fHowever,.DCNs'have been issued for all
- the plates with missing bolts to prevent additional attachments or loads.
- 3. -The two defective type N plates in the' Unit 3
'Containiment-Building were replaced;by new plates with Maxi-bolts: (documented in NCR CC-4201).

Field is in the process of verifying plates having bolts with excessive recess and missing bolts
- conditions.Inr examining the ' quality control documentations and performing l_ ultrasonic examinations.

. PROJECTED COMPLETION OF CORRECTIVE ACTION AND SUBMITTAL OF THE-
-FINAL REPORT
. Evaluation of this condition and submittal of the Final Report is forecast to be completed by May 21, 1984.
