Enclosed is our response to R. C. Lewis' June 16, 1983 letter to H. G. Parris transmitting Inspection Report Nos. 50-259/83-16,
-260/83-16, -296/83-16 regarding activities at our Browns Ferry Nuclear Plant which appeared to have been in violation of NRC regulations. We have enclosed our response to Appendix A, Notice of Violation. If you have any questions, please call Jim Domer at FTS 858-2725.

50-259/83-16, 50-260/83-16, AMD 50-296/83-16 R. C. LEWIS' LETTER TO H. G. PARRIS DATED JUNE 16, 1983 259/83-16-01 10 CFR 50, Appendix B, Criteria IX requires that, " measures shall be established to ensure that special processes, including... Nonde-structive testing and controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes, standards, specifications, criteria and other special require-ments."
TVA's Ultrasonic Inspection Procedure N-UT-25, Revision 0, Paragraph 9.4 requires that, ultrasonic scanning shall be performed at two times the primary (calibration) reference level (2X) unless other variables beyond the control of the operator preclude meaningful examination at this sensitivity.

Reasons for the Violation if Admitted

The TVA procedure was originally written to use a two node transfer technique which results in a high reference level gain setting.
In addition, the procedure states, " Scanning shall be performed at two times primary reference level (2X) unlese other variables beyond the control of the operator, e.g., inherent material noise levels, preclude meaningful examination at this sensitivity."
Therefore, when field application resulted in standing noise level which interfered with interpretation, the TVA Level III NDE examiner reduced the scanning sensitivity level

3 Corrective Steps Which Have Been Taken and the Results Achieved The procedure was changed to provide a single node transfer technique when performing half node examinations. This change reduces the primary reference level and allows scanning at two times this lower gain setting without producing standing noise levels which interfere with interpretation. This change was implemented on May 12, 1983.

Corrective Steps Which Will be Taken to Avoid Further Violations Inspection personnel have been instructed that the qualification of this procedure requires scanning at gain setting at least 6 db higher than reference level when performing examinations for detection of intergranular stress corrosion cracking.
