J. M. Farley Nuclear Plant NRC Inspection of April 11, 1983 - May 10, 1983 RE: Report Numbers 50-348/83-15 and 50-364/83-13
Dear Mr. Verrelli:
This letter refers to the violation in the subject inspection re-port which states:
"As a result.of the inspection conducted on April 11 - May 10, 1983, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violation was identified.
Technical Specification 6.8 requires in part that written pro-cedures shall be established, implemented and maintained.
Contrary to the above on May 5,1983:
1.
Evidence was found that smoking had occurred in restric-ted areas in Unit 1 auxiliary building on the 100 foot elevation in the vicinity of 1A motor driven auxiliary feedwater pump. This is contrary to the requirements of FNP-0-AP-38, "Use of Open Flame".
2.
On May 9, 1983, discrepancies were observed with the Units 1 and 2 " Key Check Out Log" in that incorrect entries were made in the signature block. Key log sheets V-110,111, and 256 were missing a total of four shift foremen initials to indicate review of log entries.

Certain as found valve position column entries were in-correct, the log sheet used for key checkout control log is not a part of any procedure..The above discrepancies are contrary to the requirement of FNP-0-SOP-0 " General-Instructions to Operating Personnel"."
Admission or Denial
-The above violation occurred as described in the subject report.
Reason for Violation This. violation was due to the following:
1.
Personnel error.
Personnel were smoking in an area not designated as a smoking area.
2.
Personnel error and procedural inadequacy.
Personnel made. improper entries in the Locked Valve Key Check-out Log. FNP-0-SOP-0.0 (General Instructions to Operating Personnel) did not provide sufficient guidance for certain special circumstances.
l Corrective Action Taken and Results Achieved Corrective action and results achieved for this violation are as follows:
1..
Appropriate signs have been posted on both the Unit 1 and Unit 2 Auxiliary Feedwater Pump Room entrance doors.
2.
An extensive investigation was conducted revealing additional errors; however, in no case were any valves 'mispositioned or unlocked when they_ should have been locked. -
Personnel associated with improper entries in the Locked Valve Key Checkout Log were individually counseled as ~ to proper log-ging of entries.
Operations Memorandum 83-07 was prepared and issued to in-L
_struct Shift Supervisors on proper key control. Also, the L
Shift Supervisors have instructed their crews on proper key l
control.

The procedure was expanded to address special key control for valves in con-tainment and transferring keys from one individual to another when it is neces-sary to continuously man and operate a valve for extended periods of time.
Erroneous referencing was deleted.
The procedure was reclassified as Safety Related and log sheets for the Locked Valve Key Checkout Log were incorporated as an appendix to the procedure.
Corrective Steps Takea to Avoid Further Violations See above. All corrective action was completed by June 23, 1983.
