Fitness for Duty Program Six Month Performance Data FITNESS FOR DUTY PROGRAM

SUMMARY

During this reporting period, approximately 55.9 percent of the population have been randomly tested. The rate of positives from random testing is 0.15 percent.

POSITIVE RESULTS:

During this reporting period, there were two positive results. One sample l

yielded positive results for marijuana.

This sample was not above the cut-off established by the NRC, but exceeded the company criteria. The second l

sample was positive for alcohol at the NRC established cut-off level.

MANAGEMENT ACTIONS:

Individuals tested positive had their access removed / denied. Management was notified and their files were annotated of the positive results and management's actions.

The two employees enrolled in follow-up testing during the last reporting period have satisfactorily completed their follow-up programs during this reporting period. One additional employee was enrolled in a follow-up testing program due to Fitness for Duty concerns.

This action was taken due to the recommendation of the Employee Assistance Program.

Four follow-up tests have been administered collectively to the employees enrolled in follow-up programs. All tests have been favorable.

As noted in an earlier reporting period, the licensee is assisting a contract company with the implementation of their Employee Assistance Program.

Seven breath tests were administered in cooperation with this effort.

Two licensee employees were brought in for for-cause testing due to Supervisory Continuous Behavior Observation Program concerns.

Both test results were negative. The employees were referred to the Employee Assistance Program for evaluation.

EVENTS REPORTED TO THE NUCLEAR REGULATORY COMMISSION (NRC):

During this reporting period, a licensed opt ator at Waterford 3 self-l referred to the Employee Assistance Program f r substance abuse. This I

event was reported to the NRC via 1-hour telephone notification to Region 4 on November 30, 1993. Follow-up information was provided in Waterford 3 letters W3F1-93-0378 dated January 5, 1994, and W3F1-94-0014 dated' February 14, 1994.

PROGRAM WEAKNESSES:

The weaknesses identified through an internal audit during the last reporting period have been addressed and corrective actions. implemented. A follow-up internal audit found the corrective actions to be satisfactory implemented.

Fitness for Duty personnel identified a weakness concerning the temperature in the freezer used for long-term storage of specimens. This weakness was documented with condition report CR-93-182.

Corrective actions have been implemented satisfactorily and the temperature is being monitored closely.
