Dear Mr. DelGeorge:
This refers to the inspection conducted by Messrs. T. Taylor, P. Prescott, and
Ms. D. Chyu of this office on June 29 through August 20, 1993.
This
inspection included a review of authorized activities for your Quad Cities
Nuclear Power Station facility. At the conclusion of the inspection, the
findings were-discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Prior to the July 9, 1993, management meeting at the Quad Cities station, we
toured the facility and were extremely disappointed with the condition of the
high pressure coolant injection (HPCI) and reactor core isolation cooling
(RCIC) pump rooms. Of particular concern is the fact that we have discussed
the performance and reliability of these important systems repeatedly with no
sustained improvements. When responding to the enclosed Notice of Violation,
recognize that past attempts to achieve long term improvements in HPCI and
RCIC performance have been ineffective. We will closely monitor your efforts
to take meaningful corrective actions.
During this inspection, certain activities appeared to be in violation of NRC
requirements, as specified in the enclosed Notice of Violation (Notice).
The condition of the HPCI and RCIC system pump rooms were in a condition not
suitably controlled. Management attention is needed to improve the condition
of these systems.
The inoperability of the toxic gas analyzer identifies a
weakness in the management of operations. That violation, combined with a
previous violation concerning corner room check valves, identified in
Inspection Report 50-254/930ll(DRP), indicates a need for aggressive
management action.
For both events, the shift engineer or shift control room
engineer failed to take appropriate actions to ensure safety related systems

were not adversely affected by planned shift activities. Additionally,
control of work and testing conducted during short outages, control of
equipment crossing contaminated area barriers, and radiological hous'ekeeping
warrant increased management attention.
