Dear Mr Keppler:

On October 8,1976, it was reported to your office that No. 3 TIP ball valve failed to auto:natically close during routine operation of the TIP System and that further investigation would be made.

Investigation during the period October 11-13, 1976, has con-fimed that the valve did not fail to close properly, but rather that cold solder connections on pins in the connecting plug attached to the valve / switch body resulted in incorrect remote position indication.

This occurrence is therefore not considered reportable. No Licensee Event Report will be issued and the Event Number assigned at the time (M-RO-76-21) will not be re-used.
