Supporting Statement for New Form 1.
Justification i.
Section 35.4 of 10 CFR Part 35 requires that applications for specific licenses to possess and use byproduct material in all medical programs be filed on Form NRC-313M.
Because of the r
diverse natures of teletherapy and other medical programs, the information requested by Form KRC-313M is not sufficient to,
describe an applicant's program without the extensive use of attachments.
The NRC expects to use, initiallya's a suggested. form, a new form specifically dedicated to teletherapy use, pfoposed Form NRC-313T. This new form should be easier for the appli-

cant to complete and should reduce the papemork burden on the i
applicant.
The new form should also be, easier for the NRC to review and should result in a savings in review time.
If the new form meets its expectations, a rule' change to 10 CFR 35.4 will require the use of the new form, ii.
The information submitted by {.he applicant for a teletherapy license will be used by the Commission to ascertain whether the applicant has the training, experience, equipment, and facilities for the use of byproduct material in teletherapy units that are adequate to protect the public health and safety.

The respondents are normally hospitals employing more than 100 persons. Approximately 95 percent of the teletherapy licenses are issued to hospitals.
