f Appendix A NOTICE OF VIOLATION Detroit - Macomb Hospitals License No. 21-01190-05 Association As a result of the inspection conducted on November 5 and 6,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
1.
License Condition No. S.B states that the maximum amount that may be possessed under this license at any one time is 2 curies of each by-product material authorized in Group III of Schedule A, Section 35.100 of 10 CFR 35.
Contrary to the above, on Tuesday morning of each week the total amount of molybdenum-99 possessed is 2.12 curies in the form of two Mo-99/Tc-99m gererators. These generators and molybdenum-99 are authorized in Group III, 10 CFR 35.100.
This is a Serverity Level V violation (Supplement VII).

License Condition No. 18 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in certain referenced applications and letters.
The referenced letter dated August 2, 1978, states in Item 2 the procedures described in Appendix D, Section 2 of the Medical Licensing Guide vill be used for calibration of the dose calibrator. Appendix D, Section 2 requires the performance of quarterly linearity tests and annual accuracy tests.
Contrary to the above, the dose calibrators at Detroit Memorial Hospital and South Macomb Hospital have not been tested for linearity nor accuracy since the requirement began on August 8, 1978.
This is a Severity Level V violation (Supplement VII).

10 CFR 35.43 states that when a misadministration involves a diagnos-tic procedure, the licensee, shall notify, in writing, the referring physician and the appropriate NRC Regional Office. These reports are due within ten days after the end of the calendar quarter in which the misadministration occurs.
Contrary to the above, a diagnostic misadministration occurred cn November 10, 1980, and as of November 6, 1981, has not been reported to NRC Region III.

10 CFR 35.14 requires that sealed calibration'or reference sources possessed pursuant to 10 CFR 35.14(e) be' tested for leakage and/or contamination at intervals not to exceed six months.
Contrary to this requirement, the 227 uCi Cs-137 and the 207 uCi Ba-133 calibration sources at Detroit Memorial Hospital have not been tested for leakage from August 4, 1978, to the date of this inspection. Also,
'the 222 uCi Cs-137 and the 198 uCi Ba-133 calibration sources at South Macomb Hospital have not been. tested for leakage from August 1978, to November 2, 1981.
