APPENDIX A NOTICE OF VIOLATION U. S. Dept. of Commerce National Marine Fisheries Service, NOAA 2725 Montlake Blvd. E Seattle, WA 98102 License flo. 46-6317-01 As a result of the inspection conducted on July 22, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified:

10 CFR 19.ll(a) requires in part that each licensee should post current copies of the license, license conditions, or documents incorporated into a license by reference, and amendments thereto.

10 CFR 19.ll(b) states that if posting of a document specified above is not practicable, the licensee may post a notice which describes the document and states where it may be examined.

Contrary to the above requirement, the license, amendments, and documents incorporated into the license by reference were neither postcJ nor was there a notice which describes the documents and states where they may be examined.

This is a Severity Level VI Violation.

10 CFR 20.203(e)(1) states that each area or room in which licensed material is used or stored and which contains any radioactive material (other than natural uranium or thorium) in an amount exceeding 10 times the quantity of such material specified in Appendix C of this part shall be conspicuously posted with a sign or signs tearing the radiation caution symbol and the words " CAUTION RADI0 ACTIVE MATERIALS".

Contrary to the above requirement, at the time of the inspection, a

" CAUTION RADIATION AREA" instead of a "CAUTI0fl RADI0 ACTIVE MATERIAL" sign n o posted on the door of the room housing the ten 250 millicurie and one 200 millicurie titanium tritide foils.

This is a Severity Level V Violation.

10 CFR 30.51(c)('.) states that records of receipt of byproduct material which must be maintained pursuant to paragraph (a) of this section shall be maintained as long as the licensee retains possession of the byproduct material and for two years following transfer or disposal of the byproduct material.

Contrary to the above requirement there is no record of receipt for the 200 millicurie Titanium Tritide foil currently in the licensee's possession.

This is a Severity Level VI Violation.

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.

The responses directed by this notice are not subject to the clearance procedures of the Office of Managenent and Pudget as required by the Paperwork Reduction Act of 1980, PL95-511.
