Gentlemen:
On January 28, 1980 it was reported to your office that a potential item under 10CFR50.55(e) existed at Plant Vogtle concerning embed plate load capacity table:. An evaluation for a significant deficiency and substantial safety hazard has been performed, and it has been concluded that this problem constitutes a significant deficiency and could have been a potential substantial safety hazard.
Bechtel Power Corporation has reported a Part 10CFR21 by letter dated July 7,1980 to Mr. Karl Seyfrit, Director of Region IV. Attached is the final report concerning the embed plate capacity load tables defi-ciency.
This response contains no proprietary information and may be placed in the NRC Public Document Room upon receipt.

JULY 7, 1980 1.0 Introduction This final report is submitted pursuant to 10CFR21 and describes actions undertaken to correct a potential substantial safety hazard /significant deficiency, associated with pipe support designs delivered to Georgia Power Company by Bechtel Power Corporation for Alvin W. Vogtle Nuclear Plant, Units 1 and 2, Docket Numbers 50-424 and 50-425.

5.0 Basic Comoonent Identification All pipe supports issued for construction on the Vogtle Project as of January 18, 1980.
This includes 5763 ASME Section III and ANSI B.31.1 (Seismic Category I, upgraded l
non-Seismic Category I and non-Seismic Category I ) pipe supports.
6.0 Deficiency Description The load capacity tables for embed plates were determined on the basis of full engagement area and no lateral loads (on the weak axis of the embed).
Application of these tables in the design of supports, in some cases, has been based on partial engagement and has allowed lateral loads in some instances.
It was considered possible that some pipe support designs released for construction might have exceeded the allowable embed loads based on partial engagement and the presence of the lateral i
loads.

7< 0 Date of Notificntion of Defect Bechtel Power Corporation identified the condition on January 25, 1980.
Bechtel's preliminary evaluation on January 28, 1980 was reported to the Georgia Power Company which in turn reported the condition under 10 CFR 50.55(e) to I&E, Region II on that date.
Bechtel continued their evaluation on January 29, 1980 and reported to I&E, Region IV on the same date that the condition may be reportable under 10 CFR 21.
8.0 Status before Corrective Actions A' total No pipe supports had been installed on Units 1 and 2.
of 5763 nuclear and non-nuclear class pipe support designs had been issued for construction.
9.0 Correceive Actions 9.1 Pipe support design issue for construction was suspended until new capacity tables for embed plates were prepared, apprcved and issued.
pipe support installation was suspended pending resolution 9.2 of design adequacy of each individual support.
9.3 Georgia Pcwer Ocmpany was notified.
A Prchlem Alert was issued to all Bechtel projects requiring 9.4 revis-cf their pipe suppcrt design for correct interpretation and implementation Of the design guide.
Twenty-Six Bechtel were alerted and responses naclear projects, present and past, Al'. projects which had used the design guide reported received.interpretation and inplementation.
All projects which correct had noc dsed tha design guide reported that their design methods had properly handled the pipe support / embed interface using basic structural design methods.
Two projects have not yet cc=menced pipe support design, but are now alerted to this incident.

I specially prepared for this review effort.
1 11.0 Redesigned Supports The extent of redesign performed during this effort falls into one or more of the following categories:

Increased cross-section of the supporting steel o
Addition of a back-up plate between the supporting steel and embed o
Support steel bracing.

12.0 Safety Implications Bechtel Power Corporation designs pipe supports in only Seismic Category I buildings, in which all supports are designed either as Seismic Category I and as upgraded non-Seismic Category I supports, or as non-Seismic Category I supports.

According to Regulatory Guide 1.29, only Seismic Category I and upgraded non-Seismic Category I supports are safety related basic components.
The redesigned Seismic Category I and upgraded non-Seismic Category I supports have been originally issued for construction and have exceeded the allowable embed 1cais c various degrees in design.
It is possible that the existing design margins may not have been sufficient to preclude inelastic deformations and prevent failure of few pipe supports under design loads.
13.0 Conclusion The inclusion Of Seismic Category I and upgraded non-Seismic Catescry I supports in the subject finding qualifies the e::istin
=cndition as a reportable deficiency under 10 CFR 21.
