Description of the Deficiency The constrs.ctor discovered that daring flush of the Standby Service Water (SSW) System, the pumps caused debris to be picked up and transferred into the SSW system.
Investigation revealed that the design prmitted debris to enter the SSW basin from the surrounding area.
Debris could potentially accumulate and clog heat exchangers in safety related systems serviced by the SSW system and/or restrict orifices in -
the SSW System piping which services safety relatod equipment. Examples of safety related systems serviced by the SSW system are the RHR heat exchangers, the HPCS, RHR, & LPCS pumps bearing coolers, the HPCS Diesel
- Generator Cooler,. _and the Standby. Diesel Generator Cooler.
This item has been determined to be related to Grand Gulf Nuclear Station only. All other Bechtel' designed nuclear power plants have a screen on the suction of the SSW pumps.
This deficiency is applicable to both Units 1 and 2.
II.
Analysis of Safety Implications Should debris enter the SSW system and clog the flow patn of the cooling water, the heat exchangers could be rendered inoperable, thus leading to oil overheating, and, ultimately, to bearing and pump failure.
Therefore, a loss of cooling to safety related equipment could occur.
Due to that possible loss of cooling to safety related equipment, we have determined this deficiency reportable under 10CFR50.55(e).
III.
Corrective Actions Taken The deficiency was caused by lack of a designed " filter" system for the j
SSW system.
Our Architect / Engineer did not anticipate the extent to i
which debris would enter the SSW basins at the time they issued the l
design for the SSW system.
As outlined in I. above, this deficiency is limited to GGNS. Corrective actions are to install pump suction screens on the SSW system pumps.
Currently, these screens are being installed, and we anticipate that installation to be completed by April 3, 1981.
