Safety-Related Piping (Units 1 and 2) ' The inspector observed welding work activities for safety-related piping as described below to determine whether applicable code and procedure require-ments were being met. The applicable code for safety-related piping is the ASE B and PV Code, Section III,1977 Edition with addenda through Winter 1977.

Welder Qualification

. ,,, , _ l ' The inspector reviewed the PPP and CB&I programs for qualification of welders and welding operators for compliance with QA procedures and ASE Code requirements. The applicable Code for welding qualification is ASE B&PV Code Section IX as invoked by GPC Specifications X2AG06 Rev. 4 and X4AZ01 Section P.1 Rev. 8.

(c) PPP procedure II-8 does not require the marking of welder qualification test assemblies with regard to welder identi-fication or assembly orientation prior to the commencement l of testing. Failure to mark qualification test assemblies , l with welder identificatiori or assembly orientation could I result in loss of control of the welder taking the test and the position in which the test was administered.

. (d) Bend specimens marked DRM had parallel coarse grind marks in the expected direction of possible fissures on the convex surface.

This condition could mask possible rejectable fissures.

Welding Filler Material Control The inspector reviewed the CB&I, PPP and GPC programs for control of welding materials to determine whether materials are being purchased, accepted, stored, and handled in accordance with QA procedures and applicable code requirements.

The inspector accompanied by a representative of the licensee noted the following. Two partable rod ovens in use, plugged in, with inoper- ' ative heating elements and containing 7018 welding electrodes.

. - . . -6- , examples of welding electrodes, (damaged and stubs) discarded in trash containers in the auxiliary building and, in a trash pile outside of the Unit I containment.

GPC procedure CD-T-14, Rev.

2, " Receipt, Storage and Control of Welding Materials" paragraph V.D.3 requires low hydrogen welding electrods to be maintained in functioning portable heated rod ovens. Paragraph V.E of CD-T-14 requires damaged electrods and electrode stubs to Le returned to properly designated weld-rod scrap containers.

The above are examples of failure to follow welding material, control procedures.

Failure to accomplish activities affecting quality in ' accordance with documented procedures is in violation of Criterion V of Appendix B to 10 CFR 50.

This Violation is assigned item No. 424, "W ld R d Control".

Steel Structures and Supports (Units 1 and 2) 1.The inspector observed welding work activities for steel structures and supports as desc'ribed below to determine whether applicable code and proce-dure requiremedf.s were being met.

The applicable code for containment fabrication is the ASME B and PV Code Section III 1974 edition with addenda through sunumer 75, and Section VIII 1974 edition with addenda through , Summer 75. The applicab,le code for stud welding is AWS D1.1-75 (Rev.1).

B031-62 1010-17 (1) With regard to the above inspection the inspector noted the following: (a) The licensee has no welding procedure listing the welding parameters (i.e., are time and current) applicable to the stud welding identified above. AWS D1.1-75, paragraph 1.3.3. ' / . ' requires welding procedure with applicable parameters for stud welding.

(b) The licensee stated that they used the parameters recommended by the manufacturer as listed on the welding machine. For the studs above the "are time" set on the machine deviated from that recommended (.85 vice.33 seconds).

r* (c) The licensee has no stud base qualification data.

The licensee at the time of this inspection had not developed a '

technique to measure " arc time".

Therefore, the licensee can not d h ensine whether the parameters used conform to the.

' - -. requirements of AWS DI.1 paragraph 4.31.6 as referenced by paragraph 1.3.3.

The above are examples of inadequate controls of stud welding. Fail-ure to establish measures to assure control of special processes is in violation of Criterion IX of Appendix B to 10 CFR 50. This Violation-

New 50.55(e) Items Prior to this inspection the licensee identified the following items

as potentially reportable items.

(1) (Open) Licensee Identified Ites 424, 425/81-02-04: "Three Inch Gate Valve Closure Problem, Level I".

On November 13, 1980, the licensee nported that the subject valves to be used in the charging system, CVCS system and safety injection system may not close at full flow. This is identified in a letter to the licensee , from Westinghouse. The Westinghouse letter indicates that it is generic.

. . _ . (2) (Open) Licensee Identified Item 424/81-02-05: " Questionable Welds . on Miscellaneous Construction Steel in the Auxiliary Building". On October 10, 1980, the licensee reported that an initial inspection of all (1212) of the subject welds indicates that approximately 100 have slag includions, laps and other defects. The licensee has started a program to determine safety significance, QA inspection involvement, and traceability of welders.
