FINAL REPORT Description of Deficiencies NCR 2246 and NCR 2259 both deal with deficiencies associated with t'he installation of safety-related pipe hangers at the ERCW pumping station.

In addition, NCR'2246 includes deficiencies associated with the installation of the discharge piping for ERCW pumps NB and PB at the ERCW pumping station.

The cause of the deficiencies associated with the installation of pipe hangers is the result of minor errors in TVA Division of Engineering Design (EN DES) issued drawings and craft personnel failing to comply with issued EN DES drawings.

The cause of the deficiencies associated with the location of the discharge piping is the result of embedded pipes which interface with the discharge piping extending past their location requirements.

Safety Icplications Had this condition gone uncorrected, possible failure of the pipe hangers may have occurred during a seismic event caused by bolt failure, rotation of the supports and plates by 90 degrees, and the likelihood of concrete failure because of inadequate bolt spacing.

There is r.o safety implication identified with the discharge piping.

Corrective Action An analysis has been performed to justify the "as-installed" condition of all pipe hangers. Those hangers which were determined to be unacceptable were reworked to satisfy the analysis requirements ensuring that there is no safety or seismic problem.

A review was performet by design employees who. determined that the location of the discharge pipir g would not alter the original analysis and, the piping is therefore acceptable in the "as-installed" configuration.

Design drawings have be ?n revised to reflect the "as-installed" configuration.

EN DES employees have been instructed to do a more thorough job of checking drawings before their issuance, while craft employees have been instructed to adhere to issued design drawings during installation of pipe hangers.
