l Operational Readiness Assessment Team (ORAT) at Comanche Peak l

The Special Inspection Branch of NRR conducted the ORAT at the Comanche Peak Steam Electric Station (CPSES) October 16-27 to provide an-independent assessment of the construction and operational status. The ORAT inspection I

was conducted by a team of ten inspectors (five consultants, three senior residents,oneregionalinspector,andoneheadquartersinspector).

Although TV Electric has not requested an operating license yet, the inspection concluded that the plant was not ready to receive a five percent power license for basically three reasons. First, and fore-most, the team concluded that an insufficient number of systems were in the direct operational control of the operators in order to support a valid assessment of operational readiness. The second reason was the plant staff had not adequately assumed responsibility for the systems and areas under operational control and had not fully implemented operational programs and procedures as a direct result of the large amount of construction and maintenance work remaining completion. And finally, the operations and operations support programmatic readiness were not adequate because several important operational programs (e.g.,

safety evaluations for temporary modifications) had not been implemented or had significant weaknesses which precluded their effective implemen-tation.

Meeting with Federal Aviation Administration (FAA) Re: License Amendment Proposing Concourse Use of Thermal Neutron Activation (TNA) Explosive Detection Systems (EDS) at International Airports Representatives from the Office of Nuclear Material Safety and Safeguards and Office of the General Counsel met with officials from the FAA's Office of Civil Aviation Security to discuss deficiencies in the environmental report submitted in request of a license amendment for concourse (ticket counter) level examination of baggage at international airports throughout the country.

The TNA device utilizes ca11fornium-252 which emits thermal neutrons to activate the nitrogen atoms in plastic explosives. When activated nitrogen atoms are oetected, a computer attached to the system gives a warning signal that explosives in luggage are being smuggled onto an airplane.

In September 1989. FAA issued rulemaking (54 FR 36938) which requires airline carriers to purchase an estimeted 200 to 400 EDS devices to be installed in airports throughout the country.

Implementation of the rule will require NRC to begin reviewing license applications from airlines or other users of the EDS at the concourse level in airports.

Because of the potential impact of this device in numerous airports throughout the country, the staff is considering actions that may be necessary under the NEPA.

0WFN Cafeteria Construction on the One White Flint North cafeteria began this week.

Electrical, mechanical and dry wall contractors are on site working.

CONS has not received a formal schedule from GSA yet, but the contractor has informally indicated that he hopes to complete the construction in early February 1990.

1 Office of Nuclear Reactor Regulation Items of Interest Week Ending November 3,1989 San Onofre Unit No. 1 As of October 30, 1989, this unit is operating in fuel Cycle 10 with degraded

' fasteners on the reactor vessel thermal shield. Neutron noise analysis indi-cates that the thermal shield vibration level has increased 80% in the 6-8 hertz range and could be due to increased wear on the limiter keys. The increase is below the NRC reporting level but does warrant close monitoring which the-staff intends to do. The licensee has increased the monitoring frequency from weekly to daily as a result.

Crystal River Unit 3 On October 26 1989, Florida Power Corporation, the licensee for Crystal River

. Unit 3 (CR-3),, identified a scenario that is not bounded by the current small break LOCA analysis. This scenario postulates the initiating event to be a break in one of the four high pressure injection lines near the Reactor Coolant System cold leg penetration. Due to the limitations of the flow measurement nozzles, the licensee has determined that it is not possible to balance flow between the four lines, a long-standing requirement to protect against core damage. As a result of this determination, the High Pressure Injection System was declared inoperable and CR-3 has been placed in Mode 3.

Babcock & Wilcox has determined that, as long as CR-3 remains in Mode 3 and complies with certain operational limitations, protection of the core is assured without any need for operator action to balance the flow. The licensee has taken the necessary steps to ensure compliance with the proposed limitations l

and, therefore, has determined that they are no longer in an LCO. The licensee

is working with Babcock & Wilcox on an evaluation of the problems and has L

committed not to go to any higher mode until they have discussed their evalua-l tion with the staff.

This unbounded condition is similar to that identified on Arkansas Nuclear One in March 1989. A comparison of the two events will be conducted.

In the interim, the resident inspectors are monitoring the licensee's operations.

Additional information is now available on this matter. The inability to balance flow between the four high pressure injection lines was initially thought to be the result of the limitation of the flow nozzles. Actually, the limitation is not associated exclusively with the nozzles, but with the combi-nation of the nozzle and associated instrumentation. The licensee has deter-L mined that, by replacing certain transmitters and installing new low-range l

meters in the control room, the desired accuracy can be achieved. These modifi-L cations are in progress, and are expected to be completed this week.

The licensee states that the new low-range instrumentation is of high quality, but the system may not be entirely safety grade. The licensee will submit a l

JC0 this week that will describe the modifications being made and the procedural and training changes required, and will define the qualification of the new instrumentation. The plant will not enter a higher mode before NRC concurrence in the JCO.

WattsBarConstructionAssessmeg NRR completed the four weeks of the Watts Bar Broad-Based Construction Assessment and held an exit meeting with TVA on October 27, 1989. The team identified problems in two areas:

(1) General quality and condition of installed equipment and (2) the site management's lack of appreciation

'on the amount of work remaining prior to fuel load.

The team's findings in the first area include A large number of minor hardware discrepancies such as bent, loose, l

or missing bolts-and washers, broken flexible conduit, missing covers, loose or broken fittings, and damaged electrical penetration leads. Although TVA has previously identified some of the discrepancies, the team is concerned about the lack of protection of equipment from further damage.

Lack of seismic qualification for the exhanst fan units in the emergency diesel generator rooms.

Undersized vendor welds on tanks, heat exchangers, and other components.

Crack-like indications and other unacceptable indications on radiographs for several pipe fittings.

Improperly installed (loose) components on the steam generator pilot-operated relief valves.

Lack of assurance of concrete quality because of a lack of concrete I

j curing records.

l The team's findings in the second area include Uncertainty about the amount of necessary maintenance or replacement of instruments and equipment to bring the equipment out of layup.

The site's difficulty in quantifying scope of "to do" work.

More than 1000 punch list items on three systems turned over to testing.

Includes 300 discovery items for which the amount of work is unknown.

The team concluded that there is not a realistic basis for the previously projected fuel load date of December 1990.

Meeting with NUMARC to Discuss Generic Letter 89-10 On October 26, 1989, the NRC staff held a publicly-noticed meeting with NUMARC to discuss certain matters related to Generic Letter 89-10

" Safety-Related Motor-0perated Valve Testing and Surveillance." NUMARC had requested the meeting with the staff to discuss the, technical guidelines it had prepared to assist its member utilities in their response to the generic letter. At the October 26 meeting, the staff L

discussed NUMARC's concerns wi n respect to the generic letter and certain l

staff's interpretations expresd it the September 1989 pub'ic workshops l

on this subject. With respect to the NUMARC's technical guidelines for responding to the generic letter, the staff indicated at the meeting that formal agency comments could not be provided until the meeting minutes for the September 1989 workshops were completed. At NUMARC's request, the staff noted areas where the technical guidelines could be improved. Further, the staff stated that, if NUMARC so desired, written comments would be provided at a later date. During the meeting, NUMARC stated that a letter documenting the concerns relating to Generic Letter 89-10 would be sent to NRC Management in the near future. On October 27, NUMARC management met with the Acting Executive Direct for Operations and reiterated their concerns. The staff will continue to work with NUMARC l

and the individual licensees to provide assistance, where appropriate, in i

their development of acceptable programs to implement the generic letter.

Joma Prieta Earthquake of October 17. 1989 An earthquake damage review team (G. Bagchi and S.N. Hou of HRR and R.M. Kenneally of RES) visited various damage sites near San Francisco on October 23 and 24,1989.

Based on the readings of a larger number of stations tne magnitude of the earthquake has now been revised from the l

preliminary reported 6.9 up to 7.1 on the Richter scale. Sites visited l

i included: Market and Mission Street area, the elevated highway structure of the Embercadero Freeway, and the Marina District in San Francisco; Interstate Rt 880 (Nimitz Highway) elevated structure and downtown Oakland; the Mor,s Landing thermal power station with seven power genera _ ting units in Moss Landing on the Monterey Bay; the town of Watsonville where some of the highest ground motions were recorded.

A lot of damage in the downtown San Francisco area and in the Marina District in particular was due to soil liquefaction and amplification of ground motion by soils. The damage from the Loma Prieta earthquake seems to be related to site specific conditions and directionality rather than to just distance from the epicenter. The elevated highway structure in

~0akland suffered demage primarily due to lateral motion, and the severity of damage can be attributed to poor design of'the hinges at connections of upper deck columns and poor reinforcement detailing of these columns.

Damage to the residential buildings in the Watsonville area was mainly in the unreinforced masonry and foundations due to severe ground motion which was estimated to be in the acceleration range of 0.4g to 0.59 The Moss Landing power station suffered extensive (60%) damage to its 500KV switch yard with broken bus and switchgear insulators. However, the other two switch yards having equipment manufactured by different suppliers suffered little or no damage. There was no piping and mechanical equipment failure, except a raw water tank with 800,000 gal. capacity which ruptured at the bottom and buckled at the top. Bolts behaved very well - even those anchoring tall stacks. The station was designed in the late 50's and early 60's for static lateral force of about 0.14g, and may have seen considerably higher values. There were also some indications at other locations that equipment was anchored well but it failed to function after the earthquake.

The general indication is that engineered industrial facilities survived quite well, but brittle ceramic insulators failed as they have in previous events. The current NRC seismic design criteria should serve us well, provided we pay attention to equipment anchorage and perform plant walk-down to eliminate the observed potential weak spots.

Items of Interest Week Ending November 3,1989 INDUSTRIAL AND MEDICAL NUCLEAR SAFETY Fuel Cycle Safety

~ Nuclear Fuel Services. Inc. (NFS)

On October 25, 1989, NRC staff met with NFS representatives to discuss the ongoing plutonium (Pu) decommissioning project. During the meeting, the NFS representatives indicated that NFS had planned to ship some transuranic waste (greater than 100 nC1 Pu/gm) to the Department of Energy's (DOE) Idaho National Engineering Laboratory (INEL) for storage. Recently, NFS was informed by DOE that INEL cannot receive Pu waste because of a State ban on waste storage.

Because of this change, NFS plans to store the packaged Pu waste in an onsite warehouse. NFS will 1mplement a radiological surveillance program to maintain the integrity of the materials in storage. NFS will ship wastes having less than 100 nCi Pu/gm to the burial site in Barnwell, South Carolina.

NFS management has committed to continue the decommissioning project even

though it cannot dispose of some of the waste.

,5E) ano Babcock & W11cox LU5W) Facilities An unannounced safeguards inspection by a three member IAEA team began on October 26, 1989 at the GE low enriched uranium fuel fabrication facility in Wilmington,_ North Carolina. The IAEA audited the records and performed measurements on powder, pellets and rods. An announced safeguards inspection by a two member IAEA team began on October 30, 1989 at the B&W low enriched uranium fuel fabrication facility in Lynchburg, Virginia. The IAEA audited the records and performed measurements on pellets.

LOW-LEVEL WASTE MANAGEMENT Meeting with Washington Department of Health (WDOH) and U.S. Ecolony (USE)

Officials On October 25-26, 1989, NMSS staff members met with officials from WDOH and USE on matters related to licensing and regulation of the commercial low-level waste facility near Richland, WA. Tiey discussed the contents of a proposed Memorandum of Understanding (MOU) between the two regulatory agencies, clarifying the responsibilities of each with regard to Atomic Energy Act licensed material disposed of at the Hanford facility. NMSS and WDOH representatives met USE representatives at the facility where they discussed administrativo procedures and licensing and technical-issues.

l NUMARC BRC Rulemaking petition Status Report On Monday, October 30, Lynn Fairobent of NUMARC provided a status report on the NUMARC BRC rulemaking petition. Ms. Fairobent indicated that the petition is complete. However, the petition must still be approved by the NUMARC Board.

Ms. Fairobent indicated that the next NUMARC Board meeting will be held on l

November 8, in Chicago. At that time, NUMARC will consider submitting the L

petition. Ms. Fairobent indicated that she will inform the staff of the Board's decision.

NRC Meeting with Department of Energy (DOE) on Uranium Mill Tailings Remedial Action Program (UMTRAP) QA On November 1, 1989, LLWM staff met with DOE, at their request, to discuss concerns recently raised by NRC regarding quality assurance in the UMTRAP.

DOE staff presented details of their procedures for identifying construction problems and managing significant design changes during construction. Recent specific problems and their impact on proposed measures to streamline the NRC 1

staff's review of UMTRAP documentation were discussed.

A number of action I

Week Ending November 3, 1989 Division of Operational Assessment (DOA)

On November 1, headquarters and Regional emergency response personnel, participated in a full-scale exercise with TVA involving the Browns Ferry nuclear power plant.

Chairman Carr served as the Executive Team Director, The Headquarter exercise objectives were met and the overall performance was good.

The Technical Training Center (TTC) staff conducted special reactor simulator training October 22-27, 1989, for personnel from the TVA Project Division of NRR. The training was cor. ducted on the Browns Ferry Nuclear Plant simulator by TTC staff rrembers who formerly held SRO licenses on that specific plant.

This NRR request for special training was accommodated in support of Agency preparedness for a future Browns Ferry restart.

On October 27-28, the Palo Verde Diagnostic Team Leaders and Team Managers met at the AEOD offices in Bethesda, Maryland. During these meetings individual evaluation plans were finalized. The team is scheduled to arrive onsite November 6-17 for the initial phase of the evaluation.

e Office of Governmental and Public Affairs Items of Interest Week ending November 3,1989 International Programs Dr. Summer Sun, NRR, travelled to the Jozef Stefan Institute in Ljubljana, Yugoslavia on an IAEA technical cooperation project to assist in developing core protection and core safety limits and to discuss technical details of transient analysis and LOCA analysis, including use of computer codes and interpretation of analytical results from using those codes.

inspection under the terms of the Commonwealth of Pennsylvania and NRC Low-Level Radioactive Waste Package and Transportation Inspection Subagreement.

The inspection was performed at the Beaver Valley Pcwer Station on October 16, 1989. A copy of the inspection report has been prepared and will be provided to the Region I to allow it to be officially forwarded to the licensee for review. No apparent violations were identified.

failure to perform surveys for radiation levels in unrestricted areas, failure to secure licensed material in an unrestricted area, use of material at an unauthorized location, and failures to leak test and inventory sealed sources, block and brace packages of radioactive materials during transit, have shipping papers, store shipping papers in the proper place in the transport vehicle, and failure to properly lock moisture-density gauges.

Radiation Sterilizers. Inc.(RSI) i On November 1, the State of Georgia notified Region 11 that shipping of Cs-137 capsules from the RSI facility in Decatur, GA has been haulted.

All casks have been taken out of service because they have failed tests required by the Certificate of Compliance. At the present time, 75 normal form and 84 special form capsules remain in the RSI storage pool. A schedule for cask repair has been established at this time. The activity in the water in the RSI pool remains at a low-level (3,38E-7 uCi/ml).

There is media interest in the resulting delay in cleanup of the RSI 1

f acility. Region 11 is monitoring the situation and has been discussing options with NMSS.

Nuclear Station to examine the circumstances regarding elevated l

temperatures in the Units 2 and 3 HPCI discharge lines and to evaluate the extent and significance of damage to the discharge lines' supports i

and hangers to determine whether the damage was caused by vibration or water hamer. On November 2, 1989, Region !!! upgraded the Special Team to an Augmented Inspection Team when the Team Leader reported that the l

damage was caused by water hammer and that the event may have generic r

implications.

near Blanding Utah, met on November 1, 1989, with Uranium Recovery Field j

Office staff to discuss freeboard and storage volume requirements for the newly constructed tailings cell 4A and all upstream structures. The newly constructed tailings cell, as well as all other tailings cells at White Mesa, are lined with plastic liners to control ground water contaminations freeboard restrictions are therefore necessary to assure sufficient storage to preclude breaching the structure boundary in the event of maximum credible rainfalls.

The SALP Board meeting for Arkansas Nuclear One was conducted on November 2,1989, in the Region IV office. The Board assessed the licensee's performance for the period of July 1, 1988, through September 30, 1989.

In addition to Region IV personnel, the meeting was attended by representatives of the offices of the EDO, NRR, and AEOD.

Cooper Nuclear Station Emergency Exercise On November 2, 1989, the Deputy Regional Administrator, accompanied by a DRP Section Chief, met in Auburn, Nebraska, and presented certificates to candidates for R0 and SRO licenses.

Chevron Resources Inc. Neeting Chevron Resources Inc., operator of the Mt. Taylor uranium mine near Grants, New Mexico, has requested a meeting with NRC staff to discuss with Mr. R. Auty, Mine Superintendent for their Ranger uranium mine in Australia, regulations and practices pertaining to uranium milling in the United States. This meeting was held on November 3, 1989, in the Uranium Recovery Field Office in Denver, Colorado.

discuss the conceptual redesign of th reclamation plan. This mill is currently being decommissioned. After ipproval of the reclaration plan, reclamation of the disposal site will commence.

Kerr-McGee. Cushine. Oklahoma On October 27, 1989, PNO-IV-89-63 mentioned Kerr-McGee's intent to issue a press release regarding the formerly licensed Cushing, Oklahoma site.

date, Kerr-McGee has not issued a press release.

However, on November 1 1989, the Oklahoma State Department of Health issued a pr+ss release regarding concorr expressed by Cushing residents about possible groundwater contamination, in the press release, the state and county health departnants vffered to test private water wells for contamination.

Meanwhile, Kerr-McGee has five people laying out a survey grid at the site.

Week Ending November 3, 1989 On October 30, 1989, the Acting Executive Director for Operations signed a proposed rule that would amend 10 CFR Part 34, "ASNT Certification of Industrial Radiographers," by providing license applicants the option to affirm that all of their active radiographers will be certified in radiation l

safety by the American Society for Nondestructive Testing prior to connencing duties as radiographers.

This option could be exercised in lieu of current licensing requirements to submit descriptions of planned initial radiation safety training and qualification procedures. The intent of this proposed rulemaking is to recognize the ASNT " Certification Program for Industrial Radiography Radiation Safety Personnel" as an acceptable method of ensuring that radiographers are adequately trained in the radiation safety subjects listed in Appendix A of 10 CFR Part 34 and to encourage industrial radiography licensees to participate in the ASNT program.

This constitutes notice to the Commission that, in accordance with the rulemaking authority delegated to the EDO, the EDO has signed this proposed rule for publication in the Federal Register.
